奥氮平联合阿立哌唑治疗精神分裂症临床疗效及对患者认知功能 神经营养因子水平的影响

摘　　要：目的 探讨奥氮平联合阿立哌唑治疗精神分裂症临床疗效及对患者认知功能、神经营养因子水平的影响。方法 将70例精神分裂症患者按照随机数字表法分为研究组与对照组,各35例,两组均给予奥氮平片治疗,研究组联合阿立哌唑治疗,观察8周。治疗前后采用阳性与阴性症状量表评定两组精神症状严重程度,采用威斯康星卡片分类测验及中国修订韦氏记忆量表评定两组认知功能,比较两组血清脑源性神经营养因子、神经生长因子、神经营养因子3水平。结果 治疗后两组阳性与阴性症状量表总分及各分量表评分均显著低于治疗前(P<0.01),研究组显著低于对照组(P<0.01)。治疗后两组中国修订韦氏记忆量表即刻记忆、延迟记忆、视觉记忆、延迟视觉记忆评分及威斯康星卡片分类测验分类完成数均显著升高(P<0.01),持续错误数均显著降低(P<0.01),研究组较对照组变化更显著(P<0.05或0.01)。治疗后两组血清脑源性神经营养因子、神经生长因子、神经营养因子3水平均显著高于治疗前(P<0.01),研究组显著高于对照组(P<0.05或0.01)。结论 奥氮平联合阿立哌唑治疗有助于改善精神分裂症患者的精神症状,改善认知功能,提高血清神经营养因子水平,值得临床进一步推广。Objective To investigate the clinical efficacy of olanzapine combined with aripiprazole in the treatment of schizophrenia and its effects on cognitive function and neurotrophic factor levels.Methods A total of 70 patients with schizophrenia were divided into study group and control group according to random number table method,with 35 cases in each group.Both groups were treated with olanzapine tablets,and the study group was treated with aripiprazole for 8 weeks.Before and after treatment,the positive and negative symptoms scale(PANSS)was used to evaluate the severity of mental symptoms in the two groups,and the Wisconsin card sorting test(WSCT)and Chinese-revised wechsler memory scale(WMS-RC)was used to evaluate the cognitive function of the two groups.The serum levels of brain-derived neurotrophic factor(BDNF),nerve growth factor(NGF),neurotrophin 3(NT-3)between the two groups were compared.Results After treatment,the total score of PANSS and the scores of each subscale in two groups were significantly lower than those before treatment(P<0.01),and the study group was significantly lower than the control group(P<0.01).After treatment,the scores of immediate memory,delayed memory,visual memory,delayed visual memory and classification completion in the two groups significantly increased(P<0.01),and the scores of continuous error significantly decreased(P<0.01).The changes in the study group were more significant than those in the control group(P<0.05 or 0.01).After treatment,the levels of serum BDNF,NGF and NT-3 in the two groups were significantly higher than those before treatment(P<0.01),and the study group was significantly higher than the control group(P<0.05 or 0.01).Conclusions Olanzapine combined with aripiprazole can improve the mental sympto-ms,cognitive function and serum neurotrophic factor levels of patients with schizophrenia,which is worthy of further clinical promotion.
