丙戊酸钠联合非典型抗精神病药物治疗对男性双相情感障碍患者糖脂代谢及体质量指数的影响

摘　　要：目的 探讨丙戊酸钠联合非典型抗精神病药物治疗对男性双相情感障碍患者糖脂代谢及体质量指数的影响。方法 将90例男性双相情感障碍患者按照治疗方法不同分为利培酮组、喹硫平组、奥氮平组,各30例,3组均给予丙戊酸钠治疗,利培酮组联合利培酮治疗,喹硫平组联合喹硫平治疗,奥氮平组联合奥氮平治疗,观察8周。治疗前、治疗4周及8周末采用Bech-Rafaelsen躁狂量表评定3组患者病情,比较3组患者空腹血糖、总胆固醇、高密度脂蛋白、甘油三酯、低密度脂蛋白、体质量指数。结果 治疗4周及8周末3组Bech-Rafaelsen躁狂量表评分均较治疗前显著降低(P<0.01),治疗4周末3组间比较差异有统计学意义(P<0.05),治疗8周末3组间比较差异无统计学意义(P>0.05)。治疗4周末3组总胆固醇、体质量指数水平比较差异有统计学意义(P<0.05),治疗8周末空腹血糖、总胆固醇、高密度脂蛋白、体质量指数水平比较差异有统计学意义(P<0.05)。治疗前后奥氮平组总胆固醇、甘油三酯、体质量指数水平比较差异有统计学意义(P<0.05或0.01),喹硫平组低密度脂蛋白、体质量指数水平比较差异有统计学意义(P<0.05)。结论 丙戊酸钠联合非典型抗精神病药物治疗男性双相情感障碍患者效果显著,影响患者体质量及糖脂代谢,奥氮平起效较为迅速,利培酮与喹硫平对体质量及糖脂代谢影响相对较轻。Objective To investigate the effect of sodium valproate combined with atypical antipsychotics on glucose and lipid metabolism and body mass index in male patients with bipolar disorder.Methods A total of 90 male patients with bipolar disorder were divided into risperidone group,quetiapine group and olanzapine group according to different treatment methods,with 30 cases in each group.All three groups were treated with sodium valproate,risperidone group combined with risperidone,quetiapine group combined with quetiapine,and olanzapine group combined with olanzapine for 8 weeks.Before treatment,4 weeks and 8 weeks after treatment,the Bech-Rafaelsen mania rating scale(BRMS)was used to evaluate the condition of the three groups of patients.The fasting blood sugar(FBS),total cholesterol(TC),high-density lipoprotein(HDL),triglyceride(TG),low density lipoprotein(LDL)and body mass index(BMI)of the three groups were compared.Results The BRMS scores of the three groups at the end of the 4th and 8th week of treatment were significantly lower than those before treatment(P<0.01).There were significant differences among the three groups at the end of the 4th week of treatment(P<0.05),and there were no significant differences among the three groups at the end of the 8th week of treatment(P>0.05).There were significant differences in the levels of TC and BMI among the three groups at the end of the 4th week after treatment(P<0.05).There were significant differences in the levels of FBS,TC,HDL and BMI at the end of the 8th week of treatment(P<0.05).There were significant differences in the levels of TC, TG and BMI in the olanzapine group before and after treatment ( P <0.05 or 0.01).There were significant differences in the levels of LDL and BMI in the quetiapine group ( P <0.05). Conclusions Sodium valproate combined with atypical antipsychotics has a significant effect in the treatment of male patients with bipolar disorder, which affects the body mass and glucose and lipid metabolism of the patients.
