摘　　要：目的探讨不同剂量的西甲硅油对超声内镜诊断直肠子宫内膜异位症图像质量的影响的研究。方法将246例怀疑子宫内膜异位症累及直肠的女性患者,随机分成A、B、C组,每组82例。通过直肠行超声内镜对子宫内膜异位症病变的诊断并评价累及肠壁的层次和深度的情况。A组单纯服用复方聚乙二醇电解质散,B组、C组在A组的基础上再分别服用5 mL、30 mL的西甲硅油。对各组内镜下祛泡效果及图像质量评估及分级,并评价超声内镜检查后腹胀情况。结果A、B、C组在祛泡效果、EUS图像质量及腹胀情况的组间比较差异均有统计学意义(P均<0.001)。其中B组与A组、C组与A组比较差异均有统计学意义(调整后P均<0.001),B组与C组在祛泡效果、EUS图像质量均明显优于A组,B组与C组在超声内镜后腹胀程度明显低于A组;而B组和C组在祛泡效果、EUS图像质量及术后腹胀程度均无统计学差异(调整后P=1.000)。结论西甲硅油可有效提高内镜下的祛泡效果及EUS图像质量,同时还可降低术后腹胀程度,并推荐5 mL西甲硅油用于临床。Objective To investigate the effect of different doses of simethicone on the image quality of endoscopic ultrasonography in diagnosing rectal endometriosis.Methods In 246 female patients with suspected endometriosis involving the rectum,endoscopic ultrasonography of the rectum was performed to diagnose endometriosis lesions and evaluate the level and depth of intestinal wall involvement.These patients were randomly divided into group A,group B and group C,with 82 cases in each group.Group A was given polyethylene glycol electrolyte,group B and group C were given 5 mL and 30 mL simethicone respectively on the basis of group A.The defoaming effect and image quality under endoscopy were graded,abdominal distention after endoscopic ultrasonography was recorded,and the differences among groups were compared.Results There were statistically significant differences in the defoaming effect,EUS image quality and abdominal distention in groups A,B and C(P<0.001).And there were statistically significant differences between group B and group A,and between group C and group A(P<0.001).Both groups B and C were significantly better than group A in terms of defoaming effect and EUS image quality,and the abdominal distension degree of group B and group C was significantly lower than group A after endoscopic ultrasonography.However,there were no significant differences between group B and group C in the defoaming effect,EUS image quality and postoperative abdominal distension(P=1.000).Conclusion Simethicone can effectively improve the efficacy of removing the bubble and EUS image quality,as well as reduce the degree of abdominal distension after endoscopic ultrasonography,and 5 mL simethicone is recommended for clinical use.
