摘 要:目的探讨接受他莫昔芬辅助内分泌治疗的乳腺癌患者人类抗原R(human antigen R,HuR)胞浆表达水平与生存的相关性。方法选取2005年1月至2016年12月接受他莫昔芬辅助内分泌治疗的Ⅰ~Ⅲ期乳腺癌患者105例。免疫组化方法评估胞浆HuR表达水平。SPSS 21.0软件进行统计学分析。结果HuR强表达与Ki-67增殖指数高显著相关。Kaplan-Meier生存分析显示,Ki-67指数高以及HuR表达水平强的患者DFS和OS显著更差。COX多因素预后分析显示,腋窝淋巴结转移状态(P=0.014,HR=6.845,95%CI:1.486~31.518)、Ki-67指数(P=0.038,HR=9.995,95%CI:1.141~87.577)、HuR表达水平(P=0.006,HR=6.820,95%CI:1.758~26.464)是独立的预后因素。结论在接受他莫昔芬内分泌治疗的雌激素受体阳性乳腺癌患者中,HuR胞浆高表达者预后更差,HuR可能成为逆转他莫昔芬耐药的一个靶点。Objective To study the correlation between cytoplasmic HuR level and prognosis of estrogen receptor(ER)positive breast cancer patients treated with tamoxifen.Methods A total of 105 patients with stageⅠ~ⅢER positive breast cancer was enrolled.Immunohistochemical analysis was performed to determine HuR expression level.SPSS 21.0 statistical software was used for analysis.Results Higher expression level of cytoplasmic HuR was associated with higher Ki-67 index.Kaplan-Meier analysis revealed that higher Ki-67 index and HuR expression level were significantly associated with poor DFS and OS.After the multi-variants'cox regression analysis,we found the axillary lymph node status(P=0.014,HR=6.845,95%CI:1.486~31.518),Ki-67 index(P=0.038,HR=9.995,95%CI:1.141~87.577)and cytoplasmic HuR expression level(P=0.006,HR=6.820,95%CI:1.758~26.464)were independent prognostic factors.Conclusion In ER positive breast cancer patients treated with adjuvant tamoxifen,higher cytoplasmic expression level of HuR is related to poor prognosis.
