原发性肝癌患者血清miR-934和miR-498水平变化及其诊断价值和对预后的影响

摘　　要：目的探究血清miR-934和miR-498在原发性肝癌中的诊断价值及其对预后的影响。方法选取原发性肝癌患者(PHC组)90例,肝硬化患者(LC组)90例,健康体检者(对照组)90例,收集相关临床资料,RT-PCR检测miR-934和miR-498水平。结果3组年龄、性别差异无统计学意义(P>0.05);3组的ALT、AST、AFP、miR-934、miR-498水平比较,差异均有统计学意义(P<0.05),其中PHC组ALT、AST、AFP、miR-934、miR-498水平高于对照组和LC组(P<0.05),LC组ALT、AST、AFP、miR-934、miR-498水平高于对照组(P<0.05)。ROC曲线对血清miR-934、miR-498水平及联合检测诊断原发性肝癌效能进行分析,miR-934、miR-498单独检测AUC分别为0.702、0.693,联合检测AUC为0.821,高于单独检测的AUC值。术后肝区疼痛、发热、有术后并发症、复发/转移的患者血清miR-934和miR-498水平显著高于其他患者(P<0.05)。结论miR-934、miR-498联合检测对原发性肝癌诊断阳性率高于单独检测,且miR-934、miR-498水平与患者术后恢复情况存在相关性,因此联合诊断对原发性肝癌的早期发现具有重要意义。Objective To explore the diagnostic value of serum miR-934 and miR-498 in primary liver cancer and its influence on the prognosis.Methods 90 patients with primary liver cancer were selected.There were 90 cases of cirrhosis(LC group)and 90 cases of health examination(control group).Collect the clinical data of patients,and detect the levels of miR-934 and miR-498 by RT-PCR.Results Comparing the clinical data of the 3 groups,it was found that the age and gender of the 3 groups were not statistically significant(P>0.05),and the differences in the levels of ALT,AST,AFP,miR-934,and miR-498 of the 3 groups were statistically significant(P>0.05).the levels of ALT,AST,AFP,miR-934,miR-498 in the PHC group were higher than those of the control group and LC group(P<0.05),and the levels of ALT,AST,AFP,miR-934,miR-498 in the LC group Higher than the control group(P<0.05).The ROC curve analyzes the serum levels of miR-934 and miR-498 and the efficacy of combined detection in diagnosing primary liver cancer.The AUC of miR-934 and miR-498 are 0.702 and 0.693,respectively,and the combined detection AUC was 0.821,which is higher than that of the single detection.AUC value.The serum miR-934 and miR-498 levels of patients with postoperative liver pain,fever,postoperative complications,recurrence/metastasis were significantly higher than those of other patients(P<0.05).Conclusion Diagnostic positive rate of the combined detection of miR-934 and miR-498 is higher than that of the single detection of primary liver cancer,and that the levels of miR-934 and miR-498 are correlated with the postoperative recovery of patients,Therefore,combined diagnosis is of great significance for the early detection of primary liver cancer.
