摘 要:目的研究超声造影相关参数造影剂峰值强度(PI)以及达峰时间(TTP)与甲状腺癌生物学行为之间的关系。方法将116例行超声造影检查的甲状腺结节患者纳为研究对象,以局部穿刺或手术病理穿刺检查结果作为“金标准”,116例检查者中共53例被诊断为甲状腺癌(共74个结节),其余63例被诊断为甲状腺良性结节(共80个结节)。比较良恶性甲状腺结节的超声定量参数(PI、TTP),并分析超声定量参数与甲状腺癌生物行为之间的相关性。结果恶性组结节PI以及TTP值均显著低于良性组(P<0.05);良性甲状腺结节超声造影增强反应主要表现为等增强及高增强(98.45%),恶性结节主要表现为低增强(95.50%),2组增强模式差异显著(P<0.05)。比较不同肿瘤直径、淋巴结转移、腺外浸润以及MVD计数的甲状腺恶性结节的PI及TTP,差异均有统计学意义(P<0.05),Spearman相关性分析提示,超声造影定量参数PI以及TTP与肿瘤直径、淋巴结转移、腺外浸润以及MVD计数均显著相关(P<0.05)。结论甲状腺恶性结节在超声造影增强扫描中常呈现低增强,超声造影的定量参数PI及TTP值均低于甲状腺良性结节,且超声造影还有助于评估肿瘤的生物学行为,帮助医生更为准确地掌握患者病情。Objective To investigate the correlation of contrast-enhanced ultrasound parameters peak intensity(PI)and time to peak(TTP)with biological behaviors of thyroid cancer.Methods A total of 116 patients with thyroid nodules in contrast-enhanced ultrasound examinations were included in the study.As the"gold standard",the results of local puncture or surgical pathological puncture examination showed that 53 of 116 examiners were thyroid cancer(74 nodules),and the remaining 63 cases had benign thyroid nodules(80 nodules).The quantitative ultrasound parameters(PI,TTP)of thyroid nodules were compared between benign and malignant thyroid nodules,and the correlation between the quantitative ultrasound parameters and the biological behavior of thyroid cancer was discussed.Results The PI and TTP values were significantly higher in malign group than in benign group(P<0.05).The contrast-enhanced patterns were mainly iso-and hyper-enhancement(98.45%)in benign thyroid nodules,and were hypo-enhancement(95.50%)in malignant nodules,with statistical difference between the two groups(P<0.05).PI and TTP values of thyroid malignant nodules with different tumor diameter,lymph node metastasis,extra-glandular invasion and microvessel density(MVD)count showed statistical difference(P<0.05).Spearman correlation analysis showed that the quantitative parameters PI and TTP values were significantly correlated with tumor diameter,lymph node metastasis,extra-glandular invasion and MVD count(P<0.05).Conclusion Compared with patients with benign thyroid cancer,contrast-enhanced ultrasound of patients with malignant thyroid nodules present hypo-enhancement,and lower values of PI and MVD,moreover,contrast-enhanced ultrasound can reflect the biological behavior of the tumor,which is of great value in clinical assessment.
