Ⅰ~Ⅱ期胃癌淋巴结转移与其病理特点的关系

摘　　要：目的分析Ⅰ~Ⅱ期胃癌淋巴结转移与其病理特点的关系。方法选择Ⅰ~Ⅱ期胃癌患者78例。问卷调查患者年龄、性别,调查患者病例资料,收集患者的临床分期(TNM)、浸润深度、肿瘤大小、组织类型、分级、淋巴结转移资料。结果78例Ⅰ~Ⅱ期胃癌患者中淋巴结转移20例,未转移58例。浸润深度:黏膜内癌52例,黏膜下癌26例。≥2 cm胃癌40例,<2 cm胃癌38例;病理类型:管状腺癌30例、低分化腺癌27例、印戒细胞癌10例、混合型11例。胃癌浸润深度、肿瘤大小、组织学类型、分级与淋巴结转移关系密切(P<0.05)。Logistic回归分析结果显示:胃癌浸润深度、肿瘤大小、病理类型是淋巴结转移的危险因素(P<0.05)。结论Ⅰ~Ⅱ期胃癌浸润深度、肿瘤大小和病理类型与淋巴结转移关系密切,应重视浸润深度至黏膜下癌、≥2 cm的胃癌患者。Objective To investigate the relationship between the depth of infiltration and lymph node metastasis and various pathological features of stageⅠ~Ⅱgastric cancer.Methods 78 patients with stageⅠ~Ⅱgastric cancer were selected.Clinical data including age,gender,clinical stage(TNM),depth of infiltration,tumor size,tissue type,grading,and lymph node metastasis.Results Among 78 patients with stageⅠ~Ⅱgastric cancer,20 patients had lymph node metastasis and 58 patients had no metastasis.Among the different depth of infiltration,there were 52 cases of intramucosal carcinoma and 26 cases of submucosal carcinoma.The tumor size was≥2 cm in 40 cases and<2 cm in 38 cases.The tissue types were tubular adenocarcinoma in 30 cases,poorly differentiated adenocarcinoma in 27 cases,signet-ring cell carcinoma in 10 cases and mixed type in 11 cases.Correlation analysis showed that depth of invasion,tumor size,tissue type,and grade were closely related to lymph node metastasis(P<0.05).Logistic regression analysis showed that the depth of gastric cancer invasion,tumor size,and tissue type were risk factors for lymph node metastasis(P<0.05).Conclusion The invasion depth,tumor size,and tissue type of stageⅠ~Ⅱgastric cancer are closely related to lymph node metastasis,and gastric cancer patients with submucosal gastric cancer and tumor size≥2 cm are at high risk.
