摘 要:目的 探讨乳腺癌患者癌组织P16蛋白、β-连环蛋白(β-catenin)、P53蛋白表达与人乳头瘤病毒(HPV)感染的关系。方法 采用免疫组织化学法检测126例乳腺癌组织及癌旁正常组织、85例乳腺纤维瘤组织中P16蛋白、β-catenin以及P53蛋白的表达,并分析其与HPV感染、乳腺癌临床病理特点及预后的相关性。结果 HPV、β-catenin和P53蛋白在乳腺癌组织中的表达高于乳腺纤维瘤组织和癌旁正常组织,P16蛋白在乳腺癌组织中的表达低于乳腺纤维瘤组织和癌旁正常组织(P<0.05)。69例出现淋巴结转移的乳腺癌患者中,HPV阳性率为75.36%,高于未出现淋巴结转移的乳腺癌患者(P<0.05)。HPV感染与肿瘤分期以及是否出现淋巴结转移有关(P<0.05),P16、β-catenin、P53蛋白表达与乳腺癌患者肿瘤分期以及是否出现淋巴结转移有关(P<0.05)。HPV阳性、β-catenin阳性患者2年生存率低于阴性患者,P16蛋白阳性患者高于阴性患者(P<0.05),P53蛋白阳性生存率与阴性相比,差异无统计学意义。结论 HPV感染可能通过调控P16蛋白、β-catenin蛋白的表达介导乳腺癌的发生发展,影响患者生存率及预后。OBJECTIVE To explore the relationship between expression of P16 protein, β-catenin and P53 protein in cancer tissues of breast cancer patients and human papillomavirus(HPV) infection. METHODS The expression levels of P16 protein, β-catenin and P53 in breast cancer tissues and normal adjacent tissues of 126 patients and breast fibroma tissues of 85 patients were detected by using immunohistochemistry method. The association between the expression of the proteins and the HPV infection, clinicopathological characteristics of breast cancer and prognosis was observed. RESULTS The expression levels of HPV, β-catenin and P53 protein were higher in the breast cancer tissues than in the breast fibroma tissues and normal adjacent tissues, while the expression level of P16 protein was lower in the breast cancer tissues than in the breast fibroma tissues and normal adjacent tissues(P<0.05). Among 69 breast cancer patients with lymphatic metastasis, the positive rate of HPV was 75.36%, higher than that of the patients without lymphatic metastasis(P<0.05). The HPV infection was associated with the tumor stage and lymphatic metastasis(P<0.05), the expression of P16, β-catenin or P53 protein was associated with the tumor stage and lymphatic metastasis(P<0.05). The 2-year survival rate of the HPV-positive, β-catenin-positive patients was lower than that of the HPV-negative, β-catenin-negative patients, while the 2-year survival rate of the P16 protein-positive patients was higher than that of the P16-negative patients(P<0.05);there was no significant difference in the survival rate between the P53 protein-positive patients and the P53-negative patients. CONCLUSION The HPV infection may mediate the genesis and development of breast cancer by regulating the expression of P16 protein and β-catenin protein, which may affect the survival rate and prognosis of the patients.
