摘  要:目的 探讨NOD样受体家族含半胱天冬酶激活和募集域结构域5(NLRC5)基因rs289723位点多态性与妊娠期牙周感染的遗传易感性。方法 选择2020年1月-2021年3月于漯河市中心医院妇产科规律接受产检孕产妇162名为研究对象,参照标准将孕产妇分为牙周感染组41例和非牙周感染组121例。分析两组NLRC5基因rs289723位点多态性,单因素及多因素Logistics回归分析规律产检孕产妇牙周感染的危险因素。结果 牙周感染组NLRC5基因rs289723位点AA基因型、A等位基因频率高于非牙周感染组,GG基因型、G等位基因低于非牙周感染组(P<0.05),多因素Logistics回归分析结果显示,每日刷牙次数、妊娠期糖尿病、NLRC5基因rs289723位点多态性是规律产检孕产妇牙周感染的独立危险因素(P<0.05),NLRC5基因rs289723位点AA基因型牙周感染孕产妇中/重度病变占比高于GG/GA型(P<0.05),AA基因型菌斑指数、出血指数高于GG/GA型(P<0.05)。结论 妊娠期牙周感染风险增加与NLRC5基因rs289723位点突变有关,AA型为易感基因型。OBJECTIVE To explore the polymorphisms at rs289723 locus of NOD-like receptor family contains caspase activation and recruitment domains 5(NLRC5) gene and the genetic susceptibility to periodontal infection during pregnancy. METHODS A total of 162 pregnant women who received regular prenatal checkups in obstetrics and gynecology department of the Luohe Central Hospital from Jan 2020 to Mar 2021 were recruited as the study objects and were divided into the periodontal infection group with 41 cases and the no periodontal infection group with 121 cases according to related standards. The polymorphisms at rs289723 locus of NLRC5 gene were analyzed, and univariate analysis and multivariate logistic regression analysis were performed for the risk factors for the periodontal infection in the pregnant women. RESULTS The frequencies of AA genotype and A allele at rs289723 locus of NLRC5 gene were higher in the periodontal infection group than in the no periodontal infection group;the frequencies of GG genotype and G allele were lower in the periodontal infection group than in the no periodontal infection group(P<0.05). The result of multivariate logistic regression analysis showed that times of daily teeth brushing, gestational diabetes mellitus and polymorphisms at rs289723 locus of NLRC5 gene were the independent risk factors for the periodontal infection in the pregnant women(P<0.05). The percentage of moderate/severe lesions of the pregnant women with NLRC5 gene rs289723 locus AA genotype periodontal infection was higher than that of the pregnant women with GG/GA genotype(P<0.05), the plaque index and bleeding index of the pregnant women with AA genotype were higher than those of the pregnant women with GG/GA genotype(P<0.05). CONCLUSION The increase of risk of gestational periodontal infection is associated with the mutation at rs289723 locus of NLRC5 gene, and AA genotype is a susceptible genotype.
