血清miR-501和miR-195评估HPV感染宫颈癌患者预后的临床价值

摘　　要：目的 分析血清微小RNA(microRNA,miR)-501和miR-195水平评估人乳头瘤病毒(HPV)感染宫颈癌患者预后的临床价值。方法 选取2016年10月-2017年12月南阳市中心医院就诊的85例HPV感染宫颈癌患者为研究组,选取同期80例HPV未感染宫颈癌患者为对照组。对两组血清miR-501和miR-195相对表达量及研究组患者的临床特征、术后总生存期(OS)、无进展生存期(PFS)进行分析。结果 研究组血清miR-501相对表达量为(0.86±0.16),高于对照组(P<0.05),miR-195相对表达量为(0.38±0.09),低于对照组(P<0.05)。研究组临床分期为Ⅱb-Ⅲ期、低分化、发生淋巴结转移患者血清miR-501相对表达量较高,miR-195相对表达量较低(P<0.05)。miR-501高表达组患者的术后OS和PFS估计值分别为16.663月和13.618月,均低于miR-501低表达组(P<0.05),miR-195高表达组患者的术后OS和PFS估计值分别为29.680月和29.053月,均高于miR-195低表达组(P<0.05)。结论 HPV感染宫颈癌患者表现为血清miR-501表达上调和miR-195表达下调,这一病理变化与患者临床特征和术后生存期具有关联性,可作为评价病情和预测预后的辅助指标。OBJECTIVE To analyze the clinical values of serum microRNA(miR)-501 and miR-195 in assessment of prognosis of the cervical cancer patients with human papillomavirus(HPV) infection. METHODS Totally 85 cervical cancer patients with HPV infection who were treated in Nanyang Central Hospital from Oct 2016 to Dec 2017 were assigned as the study group, meanwhile, 80 cervical cancer patients without HPV infection were chosen as the control group. The relative expression levels of serum miR-501 and miR-195 of the two groups were observed, and the clinical characteristics, postoperative overall survival(OS) and progress free survival(PFS) of the study group were observed. RESULTS The relative expression level of serum miR-501 of the study group was(0.86±0.16), significantly higher than that of the control group(P<0.05);the relative expression level of miR-195 of the study group was(0.38±0.09), significantly lower thanthat of the control group(P<0.05). In the study group, the relative expression level of serum miR-501 of the patients with clinical stage of Ⅱb-Ⅲ, low degree of differentiation, and lymphatic metastasis was high, while the relative expression level of miR-195 was low. The estimated values of postoperative OS and PFS of the miR-501 high expression group were respectively 16.663 months and 13.618 months, significantly lower than those of the miR-501 low expression group(P<0.05). The estimated values of postoperative OS and PFS of the miR-195 high expression group were respectively 29.680 months and 29.053 months, significantly higher than those of the miR-195 low expression group(P<0.05). CONCLUSION The cervical cancer patients with HPV infection show up-regulation of serum miR-501 and down-regulation of miR-195, which is associated with the clinical characteristics and postoperative survival and can be used as auxiliary indexes for evaluation of illness condition and prediction of prognosis.
