摘 要:目的探讨低密度脂蛋白受体1(LPR1)基因多态性与幽门螺杆菌(Hp)感染后胃癌易感性的关联。方法2018年1月-2020年1月北京医院收治的胃癌患者180例为病例组,根据Hp免疫球蛋白(Ig)G抗体检测结果,将其分为Hp阳性组、阴性组;另选同期年龄匹配的健康体检者40名为对照组,采用聚合酶链反应(qPCR)检测LPR1 rs688、rs5925、rs11172113和rs2228671位点的基因多态性,采用单因素和多因素Logistic回归模型分析LPR1基因多态性与Hp感染后胃癌易感性的关系。结果病例组LPR1基因rs688位点CC基因型及C基因频率分布均于高于对照组(P<0.05),Logistic回归模型分析结果显示,rs688 CC基因型(OR=1.246)、携带rs688 C等位基因(OR=2.337)是影响胃癌发生的危险因素(P<0.05)。在位点rs688,Hp感染阳性组携带CC基因型频率、C基因频率高于Hp阴性组(P<0.05),Logistic回归模型分析结果显示,rs688 CC基因型(OR=3.245)、携带rs688 C等位基因(OR=3.518)是影响胃癌患者Hp感染发生的危险因素(P<0.05)。结论LPR1的rs688位点CC基因型、C等位基因是胃癌发生的易感基因,且与Hp感染对胃癌易感性有协同作用;而rs5925、rs11172113和rs2228671位点的基因多态性可能与胃癌的发病无关。OBJECTIVE To investigate the correlation between gene polymorphisms of low-density lipoprotein receptor 1(LPR1)and susceptibility of gastric cancer after Helicobacter pylori(Hp)infection.METHODS A total of 180 patients with gastric cancer admitted to Beijing Hospital from Jan.2018 to Jan.2020 were selected as the case group and were divided into Hp positive group and negative group according to the test results of Hp IgG antibody.Another 40 age-matched healthy subjects during the same period were selected as the control group.The gene polymorphisms of LPR1 at rs688,rs5925,rs11172113 and rs2228671 loci were detected by polymerase chain reaction(qPCR),and the relationship between LPR1 gene polymorphisms and gastric cancer susceptibility after Hp infection was analyzed by univariate and multivariate Logistic regression models.RESULTS The frequencies of CC genotype and C gene at rs688 locus of LPR1 gene in case group were significantly higher than those in control group(P<0.05).Logistic regression model analysis showed that CC genotype at rs688 locus(OR=1.246)and C allele(OR=2.337)were independent risk factors of gastric cancer(P<0.05).The frequencies of CC genotype and C gene at rs688 locus in Hp positive group were significantly higher than those in Hp negative group(P<0.05).Logistic regression model analysis showed that CC genotype at rs688 locus(OR=3.245)and C allele(OR=3.518)were independent risk factors for Hp infection in patients with gastric cancer(P<0.05).CONCLUSION CC genotype and C allele at rs688 locus of LPR1 were susceptible genes of gastric cancer,and had synergistic effect with Hp infection on the susceptibility of gastric cancer,while the gene polymorphisms at rs5925,rs11172113 and rs2228671 loci might not be associated with gastric cancer.
