电凝与缝合止血对腹腔镜下卵巢成熟畸胎瘤剥除术患者卵巢功能的影响

摘　　要：目的:对比分析不同止血方法(电凝止血、缝合止血)对腹腔镜下卵巢成熟畸胎瘤剥除术患者卵巢功能等方面的影响。方法:选择2019年1月~2020年3月本院妇科诊治的80例卵巢成熟畸胎瘤患者,于腹腔镜下卵巢成熟畸胎瘤剥除术治疗基础上进行随机数字表法分组止血治疗。对照组与观察组各40例患者,分别采取双极电凝止血、3-0号可吸收缝线缝合止血。比较2组患者的雌二醇(Estradiol,E_(2))、促卵泡激素(Follicle-Stimulating Hormone,FSH)、黄体生成素(Luteinizing Hormone,LH)、抗缪勒管激素(Anti-Müllerian Hormone,AMH)水平以及卵巢储备功能、手术时间、术中出血量。结果:术后3d,观察组患者的E_(2)、AMH水平明显高于对照组,FSH、LH水平均明显低于对照组,P<0.05;术后6个月,观察组患者的窦卵泡数、卵巢体积、患侧卵巢间质血流峰值卵巢储备功能指标均明显优于对照组,P<0.05;组间手术时间、术中出血量比较,观察组与对照组接近,P>0.05。结论:对比电凝止血,腹腔镜卵巢成熟畸胎瘤剥除术患者行缝合止血减少了对卵巢储备功能的影响。Objective:To compare and analyze the effects of different hemostasis methods(electrocoagulation hemostasis,suture hemostasis)on the ovarian function of patients undergoing laparoscopic mature ovarian teratoma resection.Methods:A total of 80 patients with ovarian mature teratoma who were diagnosed and treated by the gynecology department of our hospital from January 2019 to March 2020 were selected,and based on laparoscopic ovarian mature teratoma removal,they were given random number table method for grouping hemostasis treatment.40 patients in the control group and the observation group were treated with bipolar electrocoagulation for hemostasis and 3-0 absorbable sutures for hemostasis.The levels of Estradiol(Estradiol,E_(2)),Follicle-Stimulating Hormone(FSH),Luteinizing Hormone(LH),and Anti-Müllerian Hormone(AMH)were compared between the two groups as well as ovarian reserve,operation time,intraoperative blood loss.Results:3 days after operation,the E_(2) and AMH levels of the observation group were significantly higher than those of the control group,and the FSH and LH levels were significantly lower than those of the control group,P<0.05;6 months after the operation,the number of antral follicles and ovaries in the observation group The volume and peak ovarian reserve index of ovarian interstitial blood flow on the affected side were significantly better than those of the control group,P<0.05;The operation time and intraoperative blood loss between the groups were compared,and the observation group was similar to the control group,P>0.05.Conclusion:Comparing with electrocoagulation hemostasis,suture hemostasis in patients undergoing laparoscopic ovarian mature teratoma resection reduces the impact on ovarian reserve.
