摘　　要：目的 探讨基质金属蛋白酶(MMP)和乙酰肝素酶(HPSE)表达水平与急性白血病化疗期间症状群的相关性。方法 回顾性选取2017年12月到2020年12月青岛市胶州中心医院收治的86例急性白血病患者作为研究对象,所有患者均进行化疗,分析所有患者在急性白血病化疗期间出现的不同症状,并将发生率≥25%的27个症状类型应用主轴因子分析和主成分分析方法,应用方差最大正交旋转得出共同组成的症状群,其中皮肤瘙痒-腹泻症状群患者11例,口腔相关症状群患者8例,自我形象受损相关症状群患者12例,胃肠道相关症状群患者12例,头部相关症状群患者14例,神经心理不适症状群患者29例,另选取来我院体检的30名健康者作为对照组。对比不同症状群患者和对照组的MMP-9、MMP-7、MMP-2和HPSE表达水平,并分析MMP和HPSE表达水平与急性白血病化疗期间症状群的相关性。结果 86例急性白血病患者共出现的症状类型有31种,其中发生率最高的症状为出汗,占比87.21%,发生率最低的症状为腿部或手臂肿胀,占比16.28%。不同症状群患者MMP-9、MMP-7和MMP-2表达水平差异显著,差异均有统计学意义(P <0.05);不同症状群患者HPSE阴性患者与阳性患者数量比较差异显著,差异均有统计学意义(P<0.05)。Spearman相关分析结果显示:HPSE与急性白血病化疗期间症状群无明显相关性(r=0.184,P> 0.05),MMP-9、MMP-7和MMP-2与急性白血病化疗期间症状群具有显著正相关性(r=0.586、0.579、0.374,P <0.05)。结论 基质金属蛋白酶中MMP-9、MMP-7、MMP-2表达水平与急性白血病化疗期间症状群具有明显相关性,因此可以考虑应用基质金属蛋白酶表达来为白血病化疗患者不同症状表现进行靶向治疗,并依照基质金属蛋白酶表达为急性白血病患者的诊断与治疗提供参考。Objective To investigate the correlation between the expression levels of matrix metalloproteinases(MMP)and heparanase(HPSE)and the symptoms of acute leukemia during chemotherapy.Methods A total of 86 patients with acute leukemia admitted to Jiaozhou Central Hospital from December 2017 to December 2020 were retrospectively selected as the research subjects.All patients were treated with chemotherapy.The different symptoms of all patients during chemotherapy were analyzed,and the 27 symptom types with incidence rate≥25%were applied with principal component analysis and principal factor analysis,There were 11 patients with pruritus diarrhea,8 patients with oral symptoms,12 patients with self-image impairment,12 patients with gastrointestinal symptoms,14 patients with head symptoms,29 patients with neuropsychological symptoms.The expression levels of MMP-9,MMP-7,MMP-2 and HPSE in patients with different symptom groups and control group were compared,and the correlation between the expression levels of MMP and HPSE and symptom groups of acute leukemia during chemotherapy was analyzed.Results There were 31 kinds of symptoms in 86 patients with acute leukemia,of which sweating was the most common symptom,accounting for 87.21%,and swelling of legs or arms was the least common symptom,accounting for 16.28%.There were significant differences in the expression levels of MMP-9,MMP-7 and MMP-2 among patients with different symptom groups,and the differences were statistically significant(P<0.05).There was statistically significant difference in the number of patients with negative and positive HPSE among different symptom groups(P<0.05).Spearman correlation analysis showed that there was no significant correlation between HPSE and symptom group of acute leukemia during chemotherapy(r=0.184,P>0.05),while MMP-9,MMP-7 and MMP-2 were significantly positively correlated with symptom clusters during chemotherapy for acute leukemia(r=0.586,0.579,0.374,P<0.05).Conclusion The expression levels of MMP-9,MMP-7 and MMP-2 in the matrix
