摘 要:目的 分析癌组织核因子-κB(NF-κB)、血管内皮生长因子(VEGF)表达与食管癌患者临床特征及预后的相关性。方法 选取2017年1月至2018年12月在新疆医科大学附属肿瘤医院保存的178例术中切除食管癌组织及癌旁组织标本为研究对象,对组织标本中的NF-κB、VEGF表达情况、患者临床特征进行回顾性分析,对患者术后连续随访36个月,对其总生存期(OS)、无进展生存期(PFS)进行随访。结果 食管癌组织标本中NF-κB、VEGF表达阳性率分别为65. 73%(117/178)和78. 09%(139/178),均高于癌旁组织的12. 36%(22/178)和15. 73%(28/178),差异均有统计学意义(P<0.05)。癌组织NF-κB、VEGF表达阳性食管癌患者中临床分期为Ⅲ~Ⅳ期及存在淋巴结转移患者的比例均高于癌组织NF-κB、VEGF表达阴性患者,差异均有统计学意义(P<0.05)。癌组织NF-κB、VEGF表达阳性食管癌患者的OS和PFS估计值均低于癌组织NF-κB、VEGF表达阴性患者,差异均有统计学意义(P<0.05)。COX风险回归模型分析结果显示,食管癌患者OS与临床分期、淋巴结转移、NF-κB、VEGF具有相关性(P<0.05),PFS与组织学类型、临床分期、淋巴结转移、术后辅助治疗、NF-κB、VEGF具有相关性(P<0.05)。结论 食管癌组织中NF-κB和VEGF表达上调,其表达情况与患者的临床特征和预后具有相关性,可能参与食管癌的发生和进展机制。Objective To analyze the correlation among nuclear factor of kappa B(NF-κB),vascular endothelial growth factor(VEGF)expression in tumor tissue and clinical features and prognosis in patients with esophageal cancer.Methods A total of 178 specimens of esophageal cancer tissues and adjacent tissues removed during operation preserved in the Cancer Hospital Affiliated to Xinjiang Medical University from January 2017 to December 2018 were selected as the research objects.The expression of NF-κB,VEGF and the clinical characteristics of patients were analyzed retrospectively.The patients were followed up for 36 months,and their overall survival(OS)and progress free survival(PFS)were followed up.Results The positive rates of NF-κB,VEGF expression in esophageal cancer tissue samples were 65.73%(117/178)and 78.09%(139/178)respectively,which were higher than 12.36%(22/178)and 15.73%(28/178)in adjacent tissues(P<0.05).The proportions of patients with clinical stageⅢ-Ⅳand lymph node metastasis in esophageal cancer patients with positive NF-κB,VEGF expression in tumor tissue were higher than thosewith negative NF-κB,VEGF expression in tumor tissue.There were statisticallysignificant differences(P<0.05).The estimated values of OS and PFS in patients esophageal cancer patients with positive NF-κB,VEGF expression in tumor tissue werelower than thosewith negative NF-κB,VEGF expression in tumor tissue.There were statisticallysignificant differences(P<0.05).Cox risk regression model analysis showed that OS was correlated to clinical stage,lymph node metastasis,NF-κB and VEGF(P<0.05).PFS was correlated to histological type,clinical stage,lymph node metastasis,postoperative adjuvant therapy,NF-κB,VEGF(P<0.05).Conclusion Theincreasing positive rates of NF-κB and VEGF exist in esophageal cancer tissues.The expression is related to the clinical characteristics and prognosis of patients,which may be involved in the pathogenesis and progression of esophageal cancer.
