磁共振T1加权增强直方图特征对腮腺多形性腺瘤与腺淋巴瘤的鉴别诊断价值

摘　　要：目的:本研究分析了基于MR成像中T1加权(T1W)增强序列的直方图参数,探讨该方法对腮腺多形性腺瘤与腺淋巴瘤的鉴别诊断价值。方法:回顾性分析76例腮腺肿瘤患者的资料,包括多形性腺瘤组39例、腺淋巴瘤组37例,所有病例均经临床手术病理验证,76例病例术前均行MR常规平扫及增强扫描。提取出MR检查中的T1W增强图像,将T1W增强图像中显示肿瘤最大层面的图像用MaZda软件处理,沿肿瘤边缘勾画ROI并进行直方图分析,得到平均值、方差、偏度、峰度和第1、10、50、90、99百分位数等参数。比较两组间的差异,并绘制ROC曲线,以此评价T1W增强直方图在鉴别两种不同成份腮腺肿瘤中的诊断效能。结果:多形性腺瘤组与腺淋巴瘤组间方差、偏度、峰度和第90百分位数差异有统计学意义(P均<0.05),ROC曲线下面积分别为0.82、0.67、0.79、0.68(P均<0.05),用参数方差鉴别两种肿瘤的敏感度、特异度分别为67.6%和91.9%,偏度为75.7%和54.0%,峰度为78.4%和73.0%,第90百分位数为59.4%和83.8%。结论:基于磁共振T1W增强的直方图特征分析对鉴别腮腺多形性腺瘤与腺淋巴瘤具有一定价值,其中方差及峰度值在鉴别诊断中具有较高的诊断效能,可为临床诊断提供有价值的参考信息。Objective:To explore the differential diagnosis value of pleomorphic adenoma and adenolymphoma of parotid gland based on T1W enhanced histogram analysis.Methods:The data of 76 patients with parotid gland tumors were retrospectively analyzed,including 39 cases of pleomorphic adenoma and 37 cases of adenolymphoma confirmed by operation and pathology,performed by regular MR scanning and enhanced scanning before the operation. The representative slice of tumor on T1W enhanced image were processed with Mazda software. ROI was drawn along the tumor edge and histogram analysis was performed to obtain parameters including average value,variance,skewness,kurtosis and the 1st,10th,50th,90th and 99th percentiles. The differences between the parameters of two groups were compared,and the ROC curve was drawn to evaluate the diagnostic efficiency of the two kinds of tumors.Results:There were significant differences in variance,skewness,kurtosis and 90th percentile between pleomorphic adenoma group and adenolymphoma group(P<0.05),The area under ROC curve was 0.82,0.67,0.79 and 0.68(P<0.05). The sensitivity and specificity of parameter variance in differentiating the two tumors were 67.6% and 91.9%,the skewness was 75.7% and54.0%,the kurtosis was 78.4% and 73.0%,and the 90th percentile was 59.4% and 83.8%.Conclusion:There were high value in the differential diagnosis of pleomorphic adenoma and adenolymphoma of parotid gland based on T1W enhanced histogram analysis especially the two parameters of variance and kurtosis.
