摘  要:目的探究不同矫治力对口腔正畸患者龈沟液白细胞介素(IL)-1β、金属蛋白酶组织抑制因子-2(TIMP-2)、基质金属蛋白酶-2(MMP-2)、组织蛋白酶K(Cathepsin K)的表达影响。方法选择自愿参加的口腔正畸患者90例,随机分为A、B、C组,3组患者右侧上颌尖牙分别被施加0 g、100 g、150 g矫治力。在施加矫治力前0 d(T0),施加矫治力后的1 d(T1)、3 d(T2)、7 d(T3)、14 d(T4)收集右侧上颌尖牙远中龈沟液,使用酶联免疫吸附实验(ELISA)检测IL-1β、TIMP-2、MMP-2、Cathepsin K含量。结果与A组相比,B组、C组T1~T3时IL-1β浓度差异有统计学意义(P<0.05);在B组、C组内与T0时比较,T1~T3时IL-1β浓度差异有统计学意义(P<0.05),且在T2达到峰值,两组的变化趋势一致。与A组相比,B组、C组T1~T4时TIMP-2、MMP-2、Cathepsin K浓度差异有统计学意义(P<0.05);在B组、C组内与T0时比较,T1~T4时TIMP-2、MMP-2、Cathepsin K浓度差异有统计学意义(P<0.05),B组和C组患者龈沟液中TIMP-2、MMP-2、Cathepsin K含量变化趋势相似,T1时龈沟液中TIMP-2浓度开始缓慢升高,T3时达到峰值,之后开始迅速下降,T4时虽然下降幅度明显,但是仍然高于T0时。结论不同矫治力可以影响口腔正畸患者龈沟液IL-1β、TIMP-2、MMP-2、K的表达,且表达程度随时间变化,可作为监测正畸牙移动和选择最佳矫治力的指标。Objective To investigate the effects of different orthodontic forces on the expression of IL-1β,TIMP-2,MMP-2 and Cathepsin K in gingival crevicular fluid of patients undergoing orthodontic treatment.Methods A total of 90 orthodontic patients who voluntarily participated in this study were randomly divided into Group A(0 g),Group B(100 g)and Group C(150 g).The distal and middle gingival crevicular fluids were collected on the right maxillary canine 0d(T0)before treatment and 1 d(T1),3 d(T2),7 d(T3)and 14 d(T4)after treatment.The contents of IL-1β,TIMP-2,MMP-2 and Cathepsin K were assessed by ELISA.Results Compared with Group A,the IL-1βconcentrations in Group B and C on T1~T3 were significantly different(P<0.05).Compared within Groups B and C before T0,the IL-1βconcentrations on T1~T3 had significant differences(P<0.05),and reached the peak on T2,with the same change trend in the both groups.Compared with the Group A,the concentrations of TIMP-2,MMP-2,and Cathepsin K in the Group B and C on T1~T4 were significantly different(P<0.05).Compared within Group B and C before T0,the concentrations of TIMP-2,MMP-2 and Cathepsin K on T1~T4 were significantly different(P<0.05).The contents of TIMP-2,MMP-2 and Cathepsin K in gingival crevicular fluid of patients in Group B and C had similar changing trends.The concentration of TIMP-2 in gingival crevicular fluid began to slowly increase on T1,reached the peak on T3 and then rapidly decreased.Although the decreasing amplitude was significant on T4,it was still higher than that before T0.Conclusion Different orthodontic forces can affect the expression levels of IL-1β,TIMP-2,MMP-2 and Cathepsin K in gingival crevicular fluid of orthodontic patients,and the expression level changes with time,which can be used as indicators to monitor orthodontic tooth movement and select the best orthodontic force in clinical treatment.
