摘 要:目的探讨鼻腔多形性腺瘤的CT、MRI表现,以提高临床诊断的准确性,改善预后。方法回顾性分析13例经病理组织学确诊的多形性腺瘤患者的临床、CT及MRI资料,观察并评估病灶的部位、大小、形态、边界、密度,分析病灶的强化方式、强化程度。结果13例患者中鼻塞9例,鼻胀痛1例,上腭疼痛1例,无意间发现1例,嗅神经母细胞瘤术后复查发现1例。11例起源于鼻中隔,2例起源于鼻腔侧壁。肿瘤局限于鼻腔10例,2例侵犯上腭,1例侵犯上颌窦、上腭。9例行CT平扫,1例行MRI动态增强扫描,3例同时行CT、MRI动态增强扫描,CT影像上均呈软组织密度,MRI检查T_(1)WI上,2例稍低信号、1例低信号、1例等信号;在T_(2)WI上,3例不均匀高信号、1例高信号。7例行增强扫描,均为不均匀强化,其中轻度强化1例,中度强化2例,明显强化4例。结论多形性腺瘤发生于鼻腔者罕见,综合病史及CT、MRI表现有助于做出诊断。Objective To investigate the CT and MRI manifestations of pleomorphic adenoma in nasal cavity,so as to improve the accuracy of clinical diagnosis and prognosis.Methods The clinical,CT and MRI data of 13 patients with pleomorphic adenoma diagnosed by histopathology in the First Affiliated Hospital of Zhengzhou University from January 2010 to August 2021 were retrospectively analyzed.The location,size,morphology,boundary and density of the lesions were observed and evaluated,and the enhancement mode and degree of the lesions were analyzed.Results There were 6 males and 7 females,aged from 20 to 81 years,with an average age of 53.15 years.There were 9 cases of nasal obstruction,1 case of nasal distension pain,1 case of upper palate pain,1 case of accidental discovery,and 1 case of olfactory neuroblastoma after reexamination.11 cases originated from the Septum and 2 cases from the lateral wall of nasal cavity.The tumors were confined to the nasal cavity in 10 cases,involving the upper palate in 2 cases and the maxillary sinus in 1 case.Nine patients underwent plain CT scanning,one underwent plain MRI scanning and enhancement,and 3 patients underwent CT and MRI scanning at the same time.All patients showed soft tissue density on CT images.MRI examination on T_(1)WI showed 2 cases with slightly low signal,1 case with low signal,and 1 case with equal signal;on T_(2)WI,3 cases with uneven high signal,and 1 case with high signal.7 cases of enhanced scanning were presented with uneven enhancement,including mild enhancement in 1 case,moderate enhancement in 2 cases,and obvious enhancement in 4 cases.Conclusion Pleomorphic adenoma is rare in nasal cavity.Comprehensive medical history and CT and MRI findings are helpful for diagnosis.
