摘　　要：目的分析原发性肛门癌的MRI表现及TNM分期,提高对原发性肛门癌的认知和诊断能力。方法回顾性分析6例原发性肛门癌患者的临床症状和MRI表现,并复习相关文献。结果5例患者肿块中心位于肛管,2例位于齿状线下,3例位于齿状线上,并沿肛管壁环形生长形成软组织肿块;1例位于肛周,呈“菜花样”浸润生长。2例肿块发生囊变坏死,1例伴发肛瘘并浸润邻近脏器。MR T_(1)WI上肿块实性成分呈稍低信号(6例),T_(2)WI上呈等信号(4例)和混杂-高信号(2例),DWI呈高信号,ADC图呈低信号,增强扫描实性成分呈轻-中度强化,并延迟强化。肿瘤T分期:T2期4例,T3期1例,T4期1例。结论原发性肛门癌罕见,其MRI表现具有特征性。Objective To investigate the MR imaging manifestations and classification of primary anal carcinoma,and in order to improve the understanding and diagnostic ability of primary anal carcinoma.Methods Six patients with primary anal carcinoma confirmed by surgery and pathology were retrospectively collected to analyze the clinical and imaging findings data in combination with relevant literatures.Results MR showed that the center of the mass in 5 cases were located in the anal canal,2 cases were located under the dentate line,and 3 cases were located on the dentate line.The five cases formed a soft tissue mass along the wall of the anal canal.One case was located in the perianal area and showed a“cauliflower-like”infiltrating growth.2 cases had cystic necrosis,and 1 case had anal fistula and infiltrated adjacent organs.The T_(2)WI signal of 4 cases showed isointensity,and 2 cases showed mixed-high signal.The dynamic enhanced scan showed light-to-moderate enhancement and delayed enhancement.T stage of tumor:4 cases were T2 stage,1 case was T3 stage,1 case was T4 stage.Conclusion Primary anal carcinoma is rare,and its imaging manifestations can be considered as a typical feature.
