摘 要:目的探讨血清可溶性人类白细胞抗原G(sHLA-G)、α-羟丁酸脱氢酶(α-HBDH)和信号转导与转录激活因子1(STAT1)在急性髓系白血病(AML)中的表达意义及与预后的关系。方法选取温州医科大学附属萧山医院2016年6月至2019年6月收治的AML患者107例为研究对象;选取2016年6月至2019年6月于温州医科大学附属萧山医院进行健康体检志愿者100例为对照组。采集AML患者与健康体检者血清标本,以酶联免疫双抗体夹心法测定血清sHLA-G浓度,以速率法测定血清α-HBDH浓度。采集AML患者与健康体检者外周血,以实时荧光定量(qRT-PCR)法测定STAT1相对表达量。结果 AML患者sHLA-G[(13.26±2.19)μg/L]和α-HBDH[(362.17±38.47)U/L]浓度均高于对照组[(5.08±0.43)μg/L、(108.94±12.19)U/L](t=36.69、62.93,均P < 0.05)。AML患者STAT1相对表达量(3.17±0.25)高于对照组(1.01±0.01)(t=86.32,P < 0.05)。sHLA-G高表达血红蛋白(Hb)≤ 80 g/L患者例数显著高于sHLA-G低表达(χ^(2)=7.15,P < 0.05),白细胞计数(WBC)> 10×109/L患者例数显著高于sHLA-G低表达(χ^(2)=17.31,P < 0.05);α-HBDH高表达Hb ≤ 80 g/L患者例数显著高于α-HBDH低表达(χ^(2)=10.76,P < 0.05),WBC > 10×109/L患者例数显著高于α-HBDH低表达(χ^(2)=13.17,P < 0.05);STAT1高表达Hb ≤ 80 g/L患者例数显著高于STAT1低表达(χ^(2)=18.14,P < 0.05),WBC > 10×109/L患者例数显著高于STAT1低表达(χ^(2)=18.51,P < 0.05)。sHLA-G高表达患者总生存时间[(17.92±1.72)个月]、α-HBDH高表达患者总生存时间[(19.34±1.57)个月]和STAT1高表达患者总生存时间[(16.36±2.08)个月]均短于低表达患者[(28.93±1.46)个月、(27.53±1.89)个月、(30.48±1.12)个月](t=35.08、24.07、38.32,均P < 0.05)。结论 sHLA-G、α-HBDH和STAT1在AML患者中异常表达,表达越高预后越差,故可作为AML治疗及预后预测的潜在靶点,具备显著创新性和科学性。Objective To investigate the significance of expression of serum soluble HLA-G(sHLA-G),alpha-hydroxybutyrate dehydrogenase(α-HBDH)and signal transducer and activator of transcription 1(STAT1)in acute myeloid leukemia(AML)and their relationship with prognosis.Methods A total of 107 patients with AML who received treatment in Xiaoshan Hospital,Wenzhou Medical University,from June 2016 to June 2019 were included in the AML group.An additional 100 healthy controls who concurrently received physical health examination in the same hospital were included in the control group.Serum samples were collected from both patients with AML and healthy controls.Serum concentration of sHLA-G concentration was determined using the double-antibody sandwich enzyme-linked immunosorbent assay.Serumα-HBDH concentration was determined using the rate method.Peripheral blood was collected from patients with AML and healthy controls.The relative expression of STAT1 was determined by quantitative real-time polymerase chain reaction.Results sHLA-G andα-HBDH concentrations in the AML group were(13.26±2.19)μg/L and(362.17±38.47)U/L,respectively,which were significantly higher than those in the control group[(5.08±0.43)μg/L,(108.94±12.19)U/L,t=36.69,62.93,both P<0.05].The relative expression of STAT1 in the AML group was significantly higher than that in the control group[(3.17±0.25)vs.(1.01±0.01),t=86.32,P<0.05]Among patients with Hb≤80 g/L,the number of patients with high sHLA-G expression was higher than that of patients with low sHLA-G expression(χ^(2)=7.15,P<0.05).The number of patients with white blood cells>10×109/L was significantly higher than that of patients with low sHLA-G expression(χ^(2)=17.31,P<0.05).Among patients with Hb≤80 g/L,the number of patients with highα-HBDH expression was significantly higher than that of patients with lowα-HBDH expression(χ^(2)=10.76,P<0.05).The number of patients with white blood cells>10×109/L was significantly higher than that with lowα-HBDH expression(χ^(2)=13.17,P<0.05).Amo
