摘　　要：目的 分析优化诊治流程对急性缺血性卒中(AIS)患者救治时间的影响。方法 选取2016年8月至2018年12月中国人民解放军联勤保障部队第九〇八医院接受常规诊治流程的AIS患者118例,作为对照组;另选取本院2019年1月至2021年12月接受优化后诊治流程的AIS患者193例,作为观察组。2组患者均于发病4.5 h内到本院就诊并行阿替普酶静脉溶栓治疗。比较2组患者进院至开始行颅脑CT平扫(T1)、颅脑CT平扫完成至溶栓(T2)、进院至溶栓(T3)及发病至溶栓(T4)等环节所耗费的时间。结果 观察组T1、T2、T3、T4均较对照组明显缩短[(25.2±7.5)min比(37.7±7.9)min、(31.1±8.8)min比(42.3±10.5)min、(59.7±25.2)min比(83.1±36.5)min、(178.6±78.7)min比(204.3±82.3)min,均P<0.01]。结论 诊治流程的优化缩短了时间窗内AIS患者的救治时间。Objective To analyze the effect of optimizing diagnosis and treatment process on treatment time in patients with acute ischemic stroke(AIS).Methods A total of 118 AIS patients who underwent routine diagnosis and treatment process in the 908^(th) Hospital of The Joint Logistic Support Force of the People’s Liberation Army from August 2016 to December 2018 were selected as the control group.In addition,193 AIS patients who received the optimized diagnosis and treatment process from January 2019 to December 2021 were selected as the observation group.All the patients were admitted to the hospital within 4.5 hours after onset and treated with intravenous thrombolysis with alteplase.The door-to-CT time(T1),CT-to-needle time(T2),door-to-needle time(T3) and onset-to-needle time(T4) were compared between the two groups.ResultsThe T1,T2,T3 and T4 in the observation group were significantly shorter than those in the control group((25.2±7.5)min vs(37.7±7.9)min,(31.1±8.8)min vs(42.3±10.5)min,(59.7±25.2)min vs(83.1±36.5)min,and(178.6±78.7)min vs(204.3±82.3)min,respectively;P<0.01).Conclusion The optimization of diagnosis and treatment process can shorten the treatment time of patients with AIS within the time window.
