心理弹性在中年良性前列腺增生患者病耻感与睡眠质量间的中介效应

摘　　要：目的分析心理弹性在中年良性前列腺增生(BPH)患者病耻感与睡眠质量间的中介效应。方法选取281例中年BPH患者采用社会影响量表(SIS)、心理弹性量表(CD-RISC)和匹兹堡睡眠质量指数量表(PSQI)进行调查。结果BPH患者病耻感总分(50.63±6.77)分,心理弹性总分(60.82±5.07)分,睡眠质量总分(7.42±2.88)分。病耻感与睡眠质量呈正相关(r=0.568,p<0.05),病耻感与心理弹性呈负相关(r=-0.407,p<0.05),心理弹性与睡眠质量呈负相关(r=-0.589,p<0.05)。病耻感对睡眠质量有直接正向效应(β=0.393,p<0.001)和间接正向效应(β=0.175);心理弹性对睡眠质量有直接负向效应(β=-0.429,p<0.001)。结论中年BPH患者睡眠质量较差,病耻感处于中等以上水平,心理弹性较低。病耻感对睡眠质量直接影响,且通过心理弹性发挥间接影响。Objective To analyze the mediating effect of psychological resilience between stigma and sleep quality in middle-aged patients with benign prostatic hyperplasia(BPH).Method A total of 281 middle-aged patients with BPH were investigated using the social impact scale(SIS),Connor-Davidson resilience scale(CD-RISC)and Pittsburgh sleep quality index(PSQI).Result The total score of stigma in BPH patients was 50.63±6.77,the total score of psychological resilience was(60.82±5.07),the total score of sleep quality was 7.42±2.88.Stigma was positively correlated with sleep quality(r=0.568,p<0.05),stigma was negatively correlated with psychological resilience(r=-0.407,p<0.05),and psychological resilience was negatively correlated with sleep quality(r=-0.589,p<0.05).Stigma had a direct positive effect(β=0.393,p<0.001)and an indirect positive effect(β=0.175)on sleep quality.Psychological resilience had a direct negative effect on sleep quality(β=-0.429,p<0.001).Conclusion The sleep quality of middle-aged BPH patients is poor,the sense of shame was above the medium level,and the psychological elasticity was low.The stigma of middle-aged patients with BPH had a direct impact on sleep quality,and indirect impact through psychological resilience.
