超声引导甲状腺肿瘤微波消融术后肿瘤区远期组织修复及血供状况的临床研究

摘　　要：目的:探讨超声引导甲状腺肿瘤微波消融术后肿瘤区远期组织的修复及血供状况。方法:对经病理证实的23例甲状腺肿瘤(其中腺瘤17例,最大径小于或等于1 cm的乳头状癌6例)于微波消融术后约半年到2年内进行消融区常规超声复查、再次活检及超声造影,并统计分析消融区及周边正常组织的峰值(Peak)、达峰时间(TP)、曲线下面积(AUC)、平均渡越时间(MTT)。结果:在术后远期,肿瘤消融区与周围正常组织常规超声无明显差异,19例再活检为正常甲状腺滤泡上皮细胞,1例为少许增生的滤泡上皮细胞,2例为纤维组织伴少许坏死细胞,1例患者拒绝再次活检。肿瘤消融区与周围正常区的Peak、TP、AUC、MTT相比,差异无统计学意义(t=-0.019、-0.121、0.06、-0.05,P>0.05)。结论:有消融指征的甲状腺肿瘤患者进行超声引导甲状腺肿瘤微波消融术后其肿瘤区远期组织的修复及血供状况良好。Objective:To investigate the long-term tissue repair and blood supply of thyroid tumors after ultrasound-guided microwave ablation.Methods:Twenty-three cases of thyroid tumors(including 17 cases of adenoma and 6 cases of papillary carcinoma with maximum diameter less than or equal to 1 cm)confirmed by pathology were reviewed by conventional ultrasound,re-biopsy and contrast-enhanced ultrasound within six months to two years after microwave ablation.The Peak value(Peak),time to Peak(TP),area under curve(AUC)and mean transit time(MTT)of the ablation area and surrounding normal tissues were statistically analyzed.Results:In the long term after operation,there was no significant difference between the ablation area and the surrounding normal tissue.19 cases of thyroid follicular epithelial cells were rebiopsies,1 case was a little proliferative follicular epithelial cells,2 cases were fibrous tissues with a few necrotic cells,and 1 patient refused to rebiopsy.There were no significant differences in Peak,TP,AUC and MTT between the ablation area and the surrounding normal area(t=-0.019,-0.121,0.06,-0.05,P>0.05).Conclusion:The long-term tissue repair and blood supply in patients with thyroid tumors indicated for ablation are good after ultrasound-guided microwave ablation of thyroid tumors.
