两种活检方法在乳腺导管原位癌术前诊断中的应用价值比较

摘　　要：目的对比超声引导下空心针穿刺活检(CNB)、麦默通(MMT)两种活检方法在乳腺导管原位癌(DCIS)术前诊断中的应用价值。方法选择2019年3月至2021年1月我院收治的156例高度疑似DCIS患者,入院时经乳腺超声检查均发现乳腺占位性病变,择期行手术治疗,术前接受超声引导下的CNB及MMT检查,以术后病理结果作为“金标准”,分析超声引导下CNB、MMT两种活检方法在DCIS术前诊断中的应用价值。结果156例高度疑似DCIS患者中,确诊DCIS 121例,非DCIS 35例;MMT检查DCIS的敏感度、准确度均高于CNB(P<0.05);CNB、MMT诊断DCIS与术后病理检查结果一致性均较好(κ=0.746、0.896)。结论超声引导下CNB、MMT诊断DCIS与术后病理检查结果一致性均较好,但MMT诊断DCIS的敏感度及准确度更高,临床可优先选择MMT对高度疑似DCIS患者进行检查,以提升诊断效能。Objective To compare the application value of ultrasound-guided core needle biopsy(CNB)and Mammotome(MMT)in the preoperative diagnosis of ductal carcinoma in situ(DCIS).Methods 156 patients with highly suspected DCIS admitted to our hospital from March 2019 to January 2021 were selected.Breast space occupying lesions were found in all patients by breast ultrasound at admission,and they received elective surgery.Ultrasound-guided CNB and MMT were performed before operation.Taking postoperative pathological results as the gold standard,the application value of CNB and MMT in the preoperative diagnosis of DCIS was analyzed.Results Of 156 patients with highly suspected DCIS,121 cases were confirmed as DCIS,and 35 cases were non-DCIS.The sensitivity and accuracy of DCIS detected by MMT were higher than that detected by CNB(P<0.05).CNB and MMT in the diagnosis of DCIS had better consistency with postoperative pathological results(κ=0.746,0.896).Conclusions Ultrasound-guided CNB and MMT in the diagnosis of DCIS had better consistency with postoperative pathological results,but MMT has higher sensitivity and accuracy in the diagnosis of DCIS.MMT can be preferentially selected for the examination of highly suspected DCIS patients so as to improve the diagnostic efficiency.
