带线钛夹在内镜黏膜下剥除术治疗大肠侧向发育型肿瘤中的应用效果

摘　　要：目的探讨带线钛夹在内镜黏膜下剥除术(ESD)治疗大肠侧向发育型肿瘤(LST)中的安全性和有效性。方法选取2018年7月至2021年12月九江市第一人民医院收治的78例乙状结肠或直肠LST患者作为研究对象,根据病灶发生部位进行分层随机化分组,分为常规组(40例)与牵引组(38例),常规组予以ESD治疗,牵引组予以带线钛夹牵引辅助ESD治疗。比较两组患者的术中剥离病变直径、病变剥除平均时间、整块切除率、完全切除率及并发症发生率。结果牵引组病变剥除平均时间短于常规组,差异有统计学意义(P<0.05)。牵引组整块切除率高于常规组,差异有统计学意义(P<0.05)。两组患者的病变完全切除率及并发症发生率比较,差异无统计学意义(P>0.05)。结论带线钛夹辅助能显著缩短结肠ESD中病变剥除时间,具有较强的安全性和有效性。Objective To investigate the safety and efficacy of dental floss traction in endoscopic submucosal dissection(ESD)of laterally developing colorectal tumors(LST).Methods A total of 78 patients with sigmoidous colon or rectal LST treated in Jiujiang No.1 People's Hospital from July 2018 to December 2021 were selected as the research objects.They were divided into routine group(40 cases)and traction group(38 cases)by stratified randomization according to the location of the lesions.The routine group was treated with ESD,and the traction group was treated with dental floss traction assisted ESD.The diameter of stripping lesion,mean time of lesion removal,en bloc resection rate,complete resection rate and complication rate were compared between the two groups.Results The mean time of lesion removal in traction group was shorter than that in routine group,the difference was statistically significant(P<0.05).The en bloc resection rate of traction group was higher than that of routine group,the difference was statistically significant(P<0.05).There were no significant differences in the complete resection rate of lesions and the incidence of related complications between the two groups(P>0.05).Conclusion The dental floss traction can significantly shorten the removal time of ESD for colorectal laterally spreading tumor,it has strong secunity and effectiveness.
