氯氮平血药浓度与精神分裂症患者临床治疗效果的关系

摘　　要：目的 分析氯氮平血药浓度与精神分裂症患者临床疗效的关系。方法 选取2020年1月—2021年12月丽水市第二人民医院86例精神分裂症患者,均采用氯氮平单一用药,测定氯氮平血药浓度,并评估临床疗效;分析氯氮平血药浓度与精神分裂症患者临床疗效的关系。结果 5例因安全性、自身原因等退出本次研究,最终81例患者纳入本次研究;与治疗1周比较,治疗2周、4周、8周时患者PANSS减分率、氯氮平血药浓度均升高,但治疗8周时,患者PANSS减分率、氯氮平血药浓度较治疗4周时降低(P<0.05);经广义估计方程分析,以治疗1周作为基础观测点,随着氯氮平血药浓度的升高,精神分裂症患者PANSS减分率呈上升趋势,且在治疗2周、4周、8周时上升显著(P<0.05)。结论 氯氮平血药浓度与精神分裂症患者临床治疗效果关系密切。Objective To analyze the relationship between clozapine plasma concentration and the clinical efficacy in patients with schizophrenia.Methods Eighty six patients with schizophrenia in Lishui Second People.s Hospital from January 2020 to December 2021 were selected as the research subjects.Clozapine was used alone,and the plasma concentration of clozapine of patients was measured.The clinical efficacy was evaluated;the relationship between clozapine plasma concentration and clinical efficacy in patients with schizophrenia was analyzed.Results Five patients withdrew from the study due to safety and their own reasons,and finally 81 patients were included in the study;compared with 1 week of treatment,the reduction rate of PANSS and the plasma concentration of clozapine of patients were increased at 2 w,4 w and 8 w of treatment.However,the PANSS score reduction rate and clozapine plasma concentration of patients at 8 w of treatment were decreased than those at 4 w of treatment(P< 0.05);according to the analysis of generalized estimation equation,taking 1 week of treatment as the basic observation point,with the increase of clozapine plasma concentration,the reduction rate of PANSS in patients with schizophrenia showed an upward trend,and was increased significantly at 2 w,4 w and 8 w of treatment(P< 0.05).Conclusion The plasma concentration of clozapine is closely related to the clinical therapeutic effect of patients with schizophrenia.
