解郁安神汤联合氟西汀治疗产后抑郁症临床研究

摘　　要：目的:观察解郁安神汤联合氟西汀治疗产后抑郁症的临床疗效,以及对患者乳汁分泌与睡眠质量的影响。方法:选取106例产后抑郁症患者,按照随机数字表法分为试验组和对照组各53例。对照组给予氟西汀治疗,试验组在对照组基础上给予解郁安神汤治疗,2组均治疗2周。比较2组治疗前后中医症状积分、汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分、乳汁分泌量及匹兹堡睡眠质量指数(PSQI)评分。结果:治疗后,2组中医症状积分均较治疗前降低(P<0.05),试验组中医症状积分低于对照组(P<0.05)。治疗后,2组HAMA、HAMD评分均较治疗前降低(P<0.05),试验组HAMA、HAMD评分均低于对照组(P<0.05)。治疗后,2组乳汁分泌量均较治疗前增多(P<0.05),试验组乳汁分泌量多于对照组(P<0.05)。治疗后,2组睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍评分均较治疗前降低(P<0.05),试验组上述PSQI各项细则评分均低于对照组(P<0.05)。结论:解郁安神汤联合氟西汀治疗产后抑郁症,能够改善患者的临床症状,减轻其抑郁程度,增加乳汁分泌量,提高睡眠质量。Objective:To observe the clinical effect of Jieyu Anshen tang combined with fluoxetine on postpartum depression and its effect on secretion of milk and sleep quality.Methods:A total of 106 patients with postpartum depression were selected and divided into the experiment group and the control group according to the random number table method,with 53 cases in each group.The control group was treated with fluoxetine,and the experiment group was additionally treated with Jieyu Anshen tang based on the treatment of the control group.Both groups were treated for two weeks.Before and after treatment,the scores of Chinese medicine symptoms,Hamilton Rating Scale for Depression(HAMD) and Hamilton Anxiety Rating Scale(HAMA),secretion of milk and scores of Pittsburgh Sleep Quality Index(PSQI) were compared between the two groups.Results:After treatment,the scores of Chinese medicine syndrome in the two groups were decreased when compared with those before treatment(P<0.05),and the Chinese medicine syndrome scores in the experiment group was lower than that in the control group(P<0.05).After treatment,the scores of HAMA and HAMD in the two groups were decreased when compared with those before treatment(P<0.05),and the above two scores in the experiment group were lower than those in the control group(P<0.05).After treatment,the secretion of milk in the two groups were increased when compared with those before treatment(P<0.05),and the secretion of milk in the experiment group was more than that in the control group(P<0.05).After treatment,the scores of sleep quality,sleep time,sleep duration,sleep efficiency,sleep disorder and daytime dysfunction in the two groups were decreased when compared with those before treatment(P<0.05),and each above score of PSQI in the experiment group were lower than those in the control group respectively(P<0.05).Conclusion:Jieyu Anshen tang combined with fluoxetine for postpartum depression can improve the clinical symptoms of patients,reduce the degree of depression,increase the secretion of mi
