摘 要:目的 探讨左心耳梳状肌分布与心源性脑卒中的相关性。方法 选取兰州大学第二医院已行经食道超声心动图(TEE)和颅脑CT或MRI检查的患者112例,分为心源性脑卒中组60例和非脑卒中组52例,TEE 135°观察梳状肌分布,将其分为6种情况:稀疏短小、稀疏高纵、浓密短小、浓密高纵、稀疏交叉、浓密交叉,评估其与心源性脑卒中的相关性。结果 两组的梳状肌浓密高纵、稀疏交叉、浓密交叉有统计学意义(P<0.05),其中浓密交叉差异性显著(P<0.01),均与心源性脑卒中的发生呈负相关;梳状肌稀疏短小、稀疏高纵、浓密短小无统计学意义(P>0.05)。梳状肌浓密高纵多见于风向袋型左心耳,梳状肌稀疏交叉、浓密交叉多见于风向袋型和菜花型左心耳。结论 LAA梳状肌分布与心源性脑卒中的发生有一定的相关性。Objective To investigate the correlation between the distribution of LAA comb muscle and cardiogenic stroke. Methods A total of 112 patients who had undergone transesophageal echocardiography(TEE) and craniocerebral CT or MRI were selected. 60 patients were divided into cardiogenic stroke group and 52 patients were divided into non-stroke group. The distribution of the comb muscle was observed at TEE 135° and divided into 6 conditions: Sparse short, sparse high longitudinal, dense short, dense high longitudinal, sparse cross and dense cross. The correlation between the distribution of the comb muscle and cardiogenic stroke was analyzed. Results There were statistically significant differences between the two groups in dense high longitudinal, sparse cross and dense cross(P<0.05). There was no significant difference in sparse short, sparse high longitudinal and dense short of the comb muscle(P>0.05),and there were statistically significant differences in dense cross(P<0.01). They were negatively correlated with cardiogenic stroke. Dense high longitudinal of comb muscle were more common in the LAA of windbag type, sparse cross and dense cross of it were more common in the left appendage of windbag type and cauffle type. Conclusions The distribution of LAA comb muscle is correlated with the occurrence of cardiogenic stroke.
