摘　　要：目的探讨免电热条件下行单孔腹腔镜卵巢畸胎瘤剥除术对卵巢成熟性畸胎瘤患者卵巢储备功能的影响。方法回顾性分析在免电热条件下行单孔腹腔镜卵巢畸胎瘤剥除术治疗的60例卵巢成熟性畸胎瘤患者的临床资料,根据卵巢畸胎瘤侧数分为单侧组51例和双侧组9例。记录患者手术情况、手术前后卵巢储备功能[抗米勒管激素(AMH)、卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇水平及窦卵泡数(AFC)]变化,并比较两组患者术后卵巢储备功能减退的发生率。结果60例患者均顺利完成手术,无中转多孔腹腔镜及开腹手术,手术时间为50.00~135.00(67.15±3.91)min,术中出血量为80.00~400.00(158.82±29.13)mL,住院时间为4.00~9.00(5.28±1.31)d。所有患者均无明显并发症。术后1个月、3个月、6个月的AMH、FSH、LH、雌二醇水平及AFC与术前比较差异均无统计学意义(均P>0.05)。单侧组与双侧组患者的FSH、LH、雌二醇、AMH水平及AFC差异均无统计学意义,两组上述指标均无随时间变化的趋势(均P>0.05)。两组术后卵巢储备功能减退发生率差异亦无统计学意义(P>0.05)。结论在免电热条件下行单孔腹腔镜卵巢畸胎瘤剥除术治疗卵巢成熟性畸胎瘤患者安全可行,对患者术后卵巢储备功能无明显影响。Objective To explore the effect of laparoendoscopic single-site ovarian teratoma stripping without electrocoagulation on ovarian reserve function in patients with mature ovarian teratoma.Methods The clinical data of 60 patients with mature ovarian teratoma undergoing laparoendoscopic single-site ovarian teratoma stripping without electrocoagulation were retrospectively analyzed,and they were assigned to unilateral group(51 cases) or bilateral group(9 cases) according to the side number of ovarian teratomata.The operative conditions,pre-and postoperative ovarian reserve function(anti-Müllerian hormone [AMH],follicle-stimulating hormone [FSH],luteinizing hormone[LH],estradiol,and antral follicle count [AFC]) changes were recorded,and the incidence rate of postoperative diminished ovarian reserve was compared between the two groups.Results Operations of the 60 patients were completed successfully without conversion to multi-site laparoscopy or open surgery.The operation duration was 50.00-135.00(67.15±3.91) min,the intraoperative bleeding volume was 80.00-400.00(158.82±29.13) mL,and the length of hospital stay was 4.00-9.00(5.28±1.31) d.No significant complication was observed in all patients.There were no statistically significant differences in 1-,3-and 6-month postoperative levels of AMH,FSH,LH,estradiol,and AFC as compared with before operation(all P>0.05).There were no statistically significant differences in levels of FSH,LH,estradiol and AMH,as well as AFC between the unilateral group and the bilateral group;in addition,there was no trend of the above indexes in the two groups over time(all P>0.05).No statistically significant difference was found in the incidence rate of postoperative diminished ovarian reserve between the two groups(P>0.05).Conclusion Conducting laparoendoscopic ovarian teratoma stripping without electrocoagulation for the treatment of patients with mature ovarian teratoma is safe and feasible,exerting no significant effect on postoperative ovarian reserve function in patients.
