不同根管封闭剂应用于慢性根尖周炎患者根管治疗中的疗效分析

摘　　要：目的分析慢性根尖周炎患者开展根管治疗时选择不同根管封闭剂的效果差异。方法选择河南科技大学第一附属医院2019年10月至2021年10月收治的86例慢性根尖周炎患者。分组方式选择随机摸红蓝球方式,其中常规组(43例)摸蓝球,试验组(43例)摸红球,将碘仿氧化锌丁香油糊剂应用于常规组患者中,将生物陶瓷类根管封闭剂iRoot SP应用于试验组患者中。针对每组患者治疗效果、各时期咀嚼效率、咬合力、松动度、出血指数、牙龈指数、并发症、肿瘤坏死因子-α水平及白细胞介素-6水平进行比较。结果试验组与常规组患者治疗有效率依次是97.62%、79.07%,差异有统计学意义(P<0.05)。同组组内治疗3 d后咀嚼效率以及咬合力高于治疗前,治疗3 d后试验组患者咀嚼效率以及咬合力高于常规组,差异有统计学意义(P<0.05)。同组组内治疗3 d后基牙松动度、出血指数以及牙龈指数均低于治疗前,治疗3 d后试验组患者基牙松动度、出血指数以及牙龈指数均低于常规组,差异有统计学意义(P<0.05)。试验组与常规组患者并发症出现率依次是6.98%、27.91%,差异有统计学意义(P<0.05)。同组组内治疗3 d后肿瘤坏死因子-α以及白细胞介素-6水平低于治疗前,治疗3 d后试验组患者肿瘤坏死因子-α以及白细胞介素-6水平低于常规组,差异有统计学意义(P<0.05)。结论慢性根尖周炎患者开展根管治疗时,使用生物陶瓷类根管封闭剂iRoot SP,患者可提高治疗效果,咀嚼功能有明显提高,牙周情况得到显著改善,减少并发症,龈沟炎症缓解,发挥一定干预作用。Objective To analyze the effect of different root canal sealers on root canal therapy in patients with chronic periapical periodontitis.Objective A total of 86 patients with chronic periapical periodontitis admitted to our hospital from October 2019 to October 2021 were selected.According to the ball-touching method,patients were divided into routine group(N=43,blue ball)and trial group(N=43,red ball).The iodoform zinc oxide-eugenol paste was applied in the routine group;the bioceramic root canal sealer iroot SP was applied to the patients in the trial group.Group contrast results were obtained through the curative effects,chewing efficiency in each period,occlusal force,tooth looseness,bleeding index,gingival index,clinical complications,tumor necrosis factor-α and interleukin-6.Results The total effective rate in the trial group and routine group was 97.62% and 79.07%(P<0.05);After 3 d of treatment,the chewing efficiency and occlusal force for two groups were significantly higher than before;the chewing efficiency and occlusal force in the trial group were significantly higher than routine group(P<0.05);After 3d of treatment,the abutment tooth tooth looseness,bleeding index and gingival index for two groups were significantly lower than before;the abutment tooth tooth looseness,bleeding index and gingival index in the trial group were even lower than routine group(P<0.05);The total complication rate in the trial group and routine group was 6.98% and 27.91%(P<0.05);After 3d of treatment,tumor necrosis factor-α and interleukin-6 levels for two groups were significantly lower than before;tumor necrosis factor-α and interleukin-6 levels in the trial group were even lower than routine group(P<0.05).Conclusion When patients with chronic periapical periodontitis carry out root canal treatment,the use of bioceramic root canal sealer iroot SP can improve the treatment effect,significantly improve the chewing function of patients,significantly improve the periodontal condition,reduce complications,alleviate gingival s
