摘　　要：目的探讨子宫颈肿瘤患者人乳头瘤病毒(HPV)-E6/E7 mRNA表达的临床意义。方法选择2019年1月至2021年12月在平顶山市妇幼保健院经阴道镜下宫颈活检的102例患者作为研究对象,所有对象行宫颈液基薄层细胞学(TCT)检查,收集患者宫颈管及宫颈口脱落细胞行HPV分型和HPV-E6/E7 mRNA定量检测,在阴道镜下行宫颈多点活检,根据活检结果,患者分为宫颈癌浸润组(21例)、宫颈上皮内瘤变(CIN)Ⅲ级组(33例)、CINⅡ级组(29例)及CINⅠ级组(19例),选择慢性宫颈炎女性40例设置为对照组,比较各组的HPV-E6/E7 mRNA的表达水平,分析HPV-E6/E7 mRNA与宫颈肿瘤病变分级的关系及其对宫颈肿瘤的诊断价值。结果HPV-E6/E7 mRNA表达水平:宫颈癌组>CINⅢ组>CINⅡ组>CINⅠ组>对照组,差异有统计学意义(P<0.05)。随TBS分级的降低,HPV-E6/E7 mRNA水平呈降低趋势,差异有统计学意义(P<0.05)。HPV-E6/E7 mRNA表达水平与疾病严重程度呈正相关(r=0.525,P=0.007)。HPV-E6/E7 m RNA鉴别诊断慢性宫颈炎与CIN的AUC为0.747(P<0.05);HPV-E6/E7 mRNA鉴别诊断宫颈癌与CIN的AUC为0.711(P<0.05)。结论HPV-E6/E7 mRNA在宫颈肿瘤患者中呈高水平表达,且表达水平随着疾病严重程度的增加而增加,检测HPV-E6/E7 mRNA的表达可鉴别诊断慢性宫颈炎、CIN及宫颈癌,对于疾病诊断及评估具有一定临床价值。Objective To investigate the clinical significance of human papillomavirus(HPV)-E6/E7 mRNA expression in patients with cervical tumor.Methods A total of 102 patients with cervical biopsy under colposcopy in Pingdingshan City Maternal and Child Health Hospital from January 2019 to December 2021 were selected as the study subjects.All subjects underwent cervical fluid based thin layer cytology(TCT)examination,and cervical canal and cervical mouth excells were collected for HPV typing and HPV-E6/E7 mRNA quantitative detection.According to the biopsy results,the patients were divided into the cervical cancer infiltration group(21 cases),the cervical intraepithelial neoplasia(CIN)Ⅲgroup(33 cases),the CINⅡgroup(29 cases)and the CINⅠgroup(19 cases).40 women with chronic cervicitis were selected as the control group.The expression levels of HPV-E6/E7 mRNA in each group were compared to analyze the relationship between HPV-E6/E7 mRNA and cervical tumor lesion grade and its diagnostic value for cervical tumor.Results HPVE6/E7 mRNA expression levels:the cervical cancer group>the CINⅢgroup>the CINⅡgroup>the CINⅠgroup>the control group,the difference was statistically significant(P<0.05).HPV-E6/E7 mRNA level decreased with the decrease of TBS grade,and the difference was statistically significant(P<0.05).HPV-E6/E7 mRNA expression was positively correlated with disease severity(r=0.525,P=0.007).The AUC of HPV-E6/E7 mRNA in differential diagnosis of chronic cervicitis and CIN was 0.747(P<0.05).The AUC of HPV-E6/E7 mRNA in differential diagnosis of cervical cancer and CIN was 0.711(P<0.05).Conclusion HPV-E6/E7 mRNA is highly expressed in cervical tumor patients,and the expression level increases with the severity of the disease.Detection of HPV-E6/E7 mRNA expression can differentiate chronic cervicitis,CIN and cervical cancer.It has certain clinical value for disease diagnosis and evaluation.
