摘 要:目的:比较影像追踪(iSCOUT)、锥形束电子计算机断层扫描(CBCT)图像及千伏级数字X射线摄影(KVDR)3种图像自动配准技术在头颈部肿瘤图像引导放射治疗摆位偏移检测中的配准精度和效率,探讨iSCOUT图像引导放射治疗定位系统的临床应用价值。方法:使用仿真人头颈模体模拟肿瘤放射治疗摆位偏移,分别通过iSCOUT、CBCT和KVDR的3种图像自动配准技术获得配准图像,与系统导入的计划CT图像或数字重建X射线摄影(DRR)图像进行自动配准得到摆位偏移,完成移床测量出配准误差,比较3种图像配准的误差及所需时间(T)。结果:摆位偏移在2 mm、4 mm和6 mm时,iSCOUT、CBCT及KVDR配准误差两两比较均无统计学差异;摆位偏移在8 mm、10 mm、12 mm和14 mm时,CBCT与KVDR相比差异具有统计学意义(F=4.664,F=2.554,F_(1)=4.209,F=2.193;P<0.05),iSCOUT与CBCT、iSCOUT与KVDR相比均无统计学差异;摆位偏移在16 mm时,CBCT与iSCOUT、CBCT与KVDR相比均有统计学差异(F=3.745,P<0.05),而iSCOUT与KVDR相比无统计学差异。配准所需时间为TiSCOUT(96.34±4.30)s,T_(CBCT)(191.89±6.25)s,T_(KVDR)(155.30±6.59)s,两两比较差异均有统计学意义(F=1105.060,P<0.01)。结论:iSCOUT图像引导放射治疗定位系统在模拟摆位偏移检测中的配准精度与CBCT和KVDR相当,但配准效率最优。Objective:To compare the registration accuracy and efficiency of three kinds of image auto-registration techniques included image scout(iSCOUT),cone beam computed tomography(CBCT)and kilo-voltage digital radiograph(KVDR)in detection of positioning displacement of image-guided radiotherapy for head and neck neoplasm,so as to explore the clinical application value of iSCOUT image-guided radiotherapy localization system.Methods:Bionic phantoms of head and neck was used to simulate the positioning displacement of radiotherapy for tumor,and the registration images of that were obtained by three kinds of image auto registration technique included iSCOUT,CBCT and KVDR.These images were conducted auto registration with the planning CT images that were imported from system or digital reconstruction radiography(DRR)images so as to obtain positioning displacement.The registration error was measured after removing bed was completed.The error and required time were compared among three kinds of image registration techniques.Results:There were no significant differences in comparison between any two means of registration errors among iSCOUT,CBCT and KVDR when the positioning displacements were 2 mm,4 mm and 6 mm.The differences between CBCT and KVDR were significant when positioning displacement were 8 mm,10 mm,12 mm and 14 mm,respectively(F=4.664,F=2.554,F=4.209,F=2.193,P<0.05),and there were not significant differences between iSCOUT and CBCT,and between iSCOUT and KVDR at the same time.There were significant differences between CBCT and iSCOUT,and between CBCT and KVDR when positioning displacement was 16 mm(F=3.745,P<0.05),while there were not significant differences between iSCOUT and KVDR.The required times of registration of three kinds of techniques were T_(iSCOUT)(96.34±4.30)s,T_(CBCT)(191.89±6.25)s and T_(KVDR)(155.30±6.59)s,respectively,and the differences of them in comparison between any two means were statistical difference(F=1105.060,P<0.01).Conclusion:The registration precision of iSCOUT image-guided radioth
