复原活血汤对下颌下腺瓣修复软组织缺损患者血流动力学及血清6-酮-前列腺素F_(1α)、血栓素B_(2)水平的影响

摘　　要：目的:研究复原活血汤对下颌下腺瓣修复软组织缺损患者血流动力学及血清6-酮-前列腺素F_(1α)(6-keto prostaglandin F_(1α),6-keto-PGF_(1α))、血栓素B_(2)(Thromboxane B_(2),TXB_(2))水平的影响。方法:选取2014年6月-2019年6月在医院行下颌下腺瓣修复口咽部肿瘤切除术后口咽部缺损患者76例,按随机数字表法分为对照组(38例,采用术后常规治疗)和研究组(38例,在对照组的基础上给予复原活血汤)。比较两组临床疗效、血液动力学指标、血清6-keto-PGF_(1α)、TXB_(2)水平变化及术后并发症发生情况。结果:研究组临床总有效率为97.37%高于对照组78.95%,差异有统计学意义(P<0.05)。治疗后,两组创面血流灌注量相对值(Blood perfusion relative,BPR)、血流速度(Blood flow velocity,BFV)水平高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清6-keto-PGF_(1α)水平高于治疗前,研究组高于对照组,差异有统计学意义(P<0.05)。两组血清TXB_(2)水平低于治疗前,研究组低于对照组,差异有统计学意义(P<0.05)。两组术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论:复原活血汤能明显改善口咽部肿瘤切除术后行下颌下腺瓣修复缺损患者的血液动力学,改善血清6-keto-PGF_(1α)、TXB_(2)水平,临床疗效确切。Objective To study the eff ect of Fuyuan Huoxue Decoction on the hemodynamics and 6-keto-PGF_(1α) and TXB_(2) levels in patients with soft tissue defects repaired by submandibular gland fl ap.Methods We selected 76 patients who underwent submandibular gland fl aps to repair oropharyngeal defects after oropharyngeal tumor resection in our hospital from June 2014 to June 2019.According to the random number table method,the patients were divided into the control group(38 cases,using conventional postoperative treatment)and the study group(38 cases,the patients were given Fuyuan Huoxue Decoction on the basis of the control group).The postoperative clinical effi cacy,hemodynamic indicators,serum 6-keto-PGF_(1α),TXB_(2) levels and the occurrence of postoperative complications were compared between the two groups.Results The total clinical eff ective rate of the study group was 97.37%,higher than that of the control group(78.95%),the diff erence was statistically signifi cant(P<0.05).After treatment,the relative value of BPR and BFV of the two groups of patients were higher than those before treatment(P<0.05).The BPR and BFV of the study group were higher than those of the control group(P<0.05).After treatment,the serum 6-keto-PGF_(1α) level of the two groups of patients was higher than those before treatment(P<0.05).The serum 6-keto-PGF_(1α) level of the study group was higher than that of the control group(P<0.05).The serum TXB_(2) level of the two groups of patients was lower than before treatment(P<0.05).And the serum TXB_(2) level of the study group was lower than that of the control group(P<0.05).There was no signifi cant diff erence in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Fuyuan Huoxue Decoction can signifi cantly improve the hemodynamics of patients undergoing submandibular gland fl ap defect repair after oropharyngeal tumor resection,and improve serum 6-keto-PGF_(1α)and TXB_(2) levels.The clinical eff ect is defi nite.
