CAD/CAM个性化基台在缺牙种植修复中的美学效果评价

摘　　要：目的:探讨CAD/CAM个性化基台在缺牙种植修复中的美学效果。方法:选取2016年7月-2019年7月唐山市口腔医院收治的51例缺牙患者为研究对象,通过随机数字表法分为观察组(n=26)和对照组(n=25)。对照组采用成品基台进行种植修复,观察组采用CAD/CAM个性化基台进行种植修复,两组均治疗3个月。比较两组治疗3个月后的龈乳头形态,对比两组治疗前、治疗3个月后的美学评分及牙周指标,统计两组治疗期间的食物嵌塞情况。结果:治疗3个月后,观察组牙龈乳头指数明显高于对照组(92.31% vs 68.00%),差异有统计学意义(P<0.05)。治疗3个月后,两组红色美学指数(PES)及白色美学指数(WES)评分较治疗前升高,观察组高于对照组,差异有统计学意义(P<0.05);而牙龈指数(GI)、牙齿菌斑指数(PLI)及龈沟出血指数(SBI)评分较治疗前降低,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗期间,观察组食物嵌塞发生率低于对照组(11.54% vs 44.00%),差异有统计学意义(P<0.05)。结论:CAD/CAM个性化基台可改善缺牙患者龈乳头形态及牙周指标,提高美学效果,同时可减少患者食物嵌塞的发生。Objective To investigate the aesthetic effect of CAD/CAM personalized abutment in implant restoration of missing teeth.Methods A total of 51 patients with missing teeth who were admitted to the department of stomatology of Tangshan stomatological hospital between July 2016 and July 2019 were selected as the research objects.They were divided into the observation group (n=26) and the control group (n=25) by random number table method.The control group used the finished abutment for implant restoration,and the observation group used the CAD/CAM personalized abutment for implant restoration.Both groups were treated for 3 months.The gingival papilla shape after 3 months of treatment in the two groups were compared;the aesthetic scores and periodontal indexes before and after 3 months of treatment in the two groups were compared;the food impaction in the two groups during treatment was counted.Results After 3 months of treatment,the gingival papilla index in the observation group was higher than the control group (92.31% vs 68.00%),the difference was statistically significant(P<0.05).After 3 months of treatment,the pink esthetic score (PES),white esthetic score (WES) scores in the two groups were higher than those before the treatment,the observation group was higher than the control group,the difference was statistically significant(P<0.05);while the gum index (GI),dental plaque index (PLI) and sulcus bleeding index (SBI) scores in the two groups were lower than those before the treatment,the observation group was lower than the control group,the difference was statistically significant(P<0.05).During treatment,the incidence of food impaction in the observation group was lower than the control group (11.54% vs 44.00%),the difference was statistically significant(P<0.05).Conclusion The CAD/CAM personalized abutment could improve the shape of the gingival papilla and periodontal indexes in patients with missing teeth,improve the aesthetic effect,and reduce the incidence of food impaction in patients.
