摘　　要：目的:探讨自酸蚀、全酸蚀粘接剂修复穿髓型楔状缺损近远期疗效及对患者附着高度、美学效果的影响。方法:选取2017年4月-2018年7月笔者医院就诊的94例(94颗)穿髓型楔状缺损患者为研究对象,根据不同粘接剂修复分为自酸蚀组和全酸蚀组,每组47例(47颗)。自酸蚀组采用自酸蚀粘接剂修复,全酸蚀组采用全酸蚀粘接剂修复。比较两组修复效果、并发症发生率、修复后牙本质敏感性、自觉疼痛程度、美学效果及修复前后牙周情况。结果:两组修复后1 d、7 d及3个月修复成功率、并发症发生率、修复后即刻、修复后12个月美学效果相比,差异无统计学意义(P>0.05);自酸蚀组修复后1、3个月牙本质敏感性优于全酸蚀组;与全酸蚀组比较,自酸蚀组修复后1 d、7 d VAS评分较低;自酸蚀组修复后6个月牙周袋探诊深度、附着高度丧失及牙龈出血指数均低于全酸蚀组,差异均有统计学意义(P<0.05)。结论:与全酸蚀粘接剂修复比较,自酸蚀粘接剂在降低穿髓型楔状缺损患者牙本质敏感性、减轻自觉疼痛程度方面具有明显优势,有助于改善牙周情况,临床应用价值更高。Objective To investigate the short-term and long-term effects of self-etch bonding and total-etch bonding in repairing wedge-shaped defect of pulp piercing type and their effects on the patient’s attachment height and aesthetic effects.Methods From April 2017 to July 2018,94 patients (94 pieces) with wedge-shaped defect of pulp piercing type in our hospital were selected.According to different adhesives,they were divided into self-etching group and total-etching group,with 47 cases (47 pieces) in each.The self-etching group was repaired with self-etch bonding,and the total-etching group was repaired with total-etch bonding.The restoration effect,complication rate,sensitivity of dentin after restoration,degree of conscious pain,aesthetic effect and periodontal condition before and after restoration were compared between the two groups.Results There were no statistically significant differences in the repair success rate at 1 d,7 d and 3 months after repair,the incidence of complications,the aesthetic effect immediately after repair,and 12 months after restoration between the two groups (P>0.05).The sensitivity of dentin in the self-etching group was better than that in the total-etching group at 1 and 3 months after restoration (P<0.05).The VAS scores of the self-etching group were lower on the 1st and 7th day after restoration compared with the total-etching group (P<0.05).The probing depth of periodontal pocket,loss of attachment height and gingival bleeding index at 6 months after restoration in the self-etching group were lower than those in the total-etching group (P<0.05).Conclusion Compared with the restoration of total-etch bonding,self-etch bonding has obvious advantages in reducing the sensitivity of the dentin and reducing the degree of pain in patients with penetrating wedge-shaped defects,and helps to improve the periodontal condition.The application value is higher.
