摘 要:目的:探讨脑脊液游离脂肪酸(FFA)水平对急性脑梗死后发生认知功能障碍的预测价值。方法:回顾性分析2018年4月至2020年12月该院收治的134例急性脑梗死患者的临床资料,入院当天检测患者脑脊液、血清FFA水平。将脑梗死后6个月内发生认知功能障碍的患者纳入发生组(n=64),未发生认知功能障碍的患者纳入未发生组(n=70),比较两组脑脊液、血清FFA水平,并采用受试者工作特征(ROC)曲线分析入院当天脑脊液、血清FFA水平对急性脑梗死后发生认知功能障碍的预测价值。结果:134例急性脑梗死患者中,发生认知功能障碍64例,发生率为47.76%;发生组入院当天脑脊液、血清FFA水平均高于未发生组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,入院当天脑脊液、血清FFA水平预测急性脑梗死后发生认知功能障碍的曲线下面积分别为0.801、0.703,脑脊液FFA水平预测认知功能障碍的灵敏度和特异度高于血清FFA水平。结论:急性脑梗死后认知功能障碍发生率较高,入院时脑脊液FFA水平对急性脑梗死后发生认知功能障碍的预测价值较高,临床可据此采取相应措施,以改善患者预后。Objective:To explore predictive value of free fatty acid(FFA) levels in cerebrospinal fluid on cognitive dysfunction after acute cerebral infarction.Methods:The clinical data of 134 patients with acute cerebral infarction admitted to the hospital from April 2018 to December 2020 were retrospectively analyzed.The serum FFA and cerebrospinal fluid FFA these patients were detected on the day of admission.The patients with cognitive dysfunction within 6 months after cerebral infarction were included in incidence group(n=64),while those patients without cognitive dysfunction were set as non-incidence group(n=70).The levels of serum FFA and cerebrospinal fluid FFA were compared.The receiver operating characteristic(ROC) curve was used to analyze the predictive values of the serum FFA and cerebrospinal fluid FFA levels on the day of admission to cognitive dysfunction after acute cerebral infarction.Results:Among the 134 patients with acute cerebral infarction,64 had cognitive dysfunction with an incidence rate of 47.76%.The levels of cerebrospinal fluid and serum FFA in the incidence group were higher than those in the non-incidence group on the day of admission,and the differences were statistically significant(P<0.05).The ROC curve analysis showed that the areas under the curve of cerebrospinal fluid and serum FFA levels on the day of admission to predict cognitive dysfunction after acute cerebral infarction were 0.801and 0.703,respectively;the sensitivity and specificity of the cerebrospinal fluid FFA levels in predicting cognitive dysfunction were also higher than those of the serum FFA levels.Conclusions:The incidence of cognitive dysfunction is higher after acute cerebral infarction.The cerebrospinal fluid FFA level at admission has a high predictive value for cognitive dysfunction after acute cerebral infarction.Therefore,the clinical measures can be taken accordingly to improve the prognosis of these patients.
