即刻种植及残根保存修复治疗前牙残根Ⅴ类缺损的疗效观察

摘　　要：目的:观察不同方案治疗前牙残根Ⅴ类缺损的临床疗效。方法:回顾性观察分析2017年1月~2020年9月于我院牙科接受即刻种植修复治疗(即刻种植修复组,n=41)及接受残根保存修复治疗(残根保存修复组,n=40)的前牙残根Ⅴ类缺损患者临床资料,对比治疗6个月后两组患者的临床疗效、修复体临床美学效果、修复体牙周组织健康状况、美观舒适度评分、咀嚼功能评分、固位功能评分及并发症发生率。结果:治疗6个月后,即刻种植修复组总有效率(90.24%)高于残根保存修复组(75.00%),差异比较无统计学意义(χ2=3.291,P=0.070);治疗6个月后,即刻种植修复组中除软组织外形评分外其余变量评分及PES平均分均较近治疗完成时显著升高(P<0.05);残根保存修复组中软组织颜色、软组织水平和牙槽突丧失评分及PES平均分均较近治疗完成时显著升高(P<0.05);治疗完成时和治疗6个月后,即刻种植修复组和残根保存修复组各变量评分及PES平均分比较差异均无统计学意义(P>0.05);治疗6个月后,即刻种植修复组牙龈和牙槽骨健康评分及单位时间内龈沟液分泌量均显著低于残根保存修复组,两组差异比较显著(P<0.05);治疗6个月后,两组美观舒适度、咀嚼功能、固位功能评分均较治疗前显著降升高(P<0.05),两组差异比较显著(P<0.05);康复过程中即刻种植修复组并发症发生率(4.88%)低于残根保存修复组(12.50%),差异比较无统计学意义(χ2=1.489,P>0.05)。结论:即刻种植和残根保存均能有效治疗前牙残根V类缺损,但即刻种植修复治疗的整体效果更优,可显著改善患者牙周健康情况,提高美观舒适度,增强咀嚼和固位功能,降低并发症发生率。Objective:To observe the clinical efficacy of different regimens in the treatment of type V residual root defects of anterior teeth.Methods:The clinical data of 81 patients with type V residual root defects of anterior teeth who received immediate implant restoration treatment(the immediate implant restoration group,n=41)and residual root preservation restoration treatment(the residual root preservation restoration group,n=40)in the department of dentistry of our hospital from January 2017 to September 2020 were retrospectively observed and analyzed.The clinical efficacy,clinical aesthetic effect of restorations,periodontal tissue health status of restorations,aesthetic comfort score,masticatory function score,retention function score and incidence rate of complications were compared between the two groups after 6 months of treatment.Results:After 6 months of treatment,the total effective rate of the immediate implant restoration group was higher than that of the residual root preservation restoration group(90.24%vs 75.00%)(χ2=3.291,P=0.070).After 6 months of treatment,the scores of other variables except for soft tissue shape and the average PES score of immediate implant restoration group were significantly increased compared with those at the completion of treatment(P<0.05).The scores of soft tissue color,soft tissue level and alveolar process loss and the average PES score in the residual root preservation restoration group were significantly higher than those at the completion of treatment(P<0.05).At the completion of treatment and 6 months after treatment,there were no statistical significance in the scores of variables and average PES score between the immediateimplant restoration group and the residual root preservation restoration group(P>0.05).After 6 months of treatment,the scoresof gingival and alveolar bone health and gingival crevicular fluid secretion per unit time of immediate implant restorationgroup were significantly lower than those of residual root preservation restoration group(P<0.05).6 mo
