摘 要:目的:评价纤维桩成型钻应用频次对粘接强度的影响。方法:48颗单根管第二前磨牙于釉牙骨质界冠方2 mm截冠,完善根管治疗后随机分为8组,每组6颗,记为T1至T8。采用6支全新直径1.5 mm纤维桩成型钻为T1组样本进行桩道预备,常规纤维桩粘接完成后将成型钻清洗干净,高温高压灭菌消毒。按T1组程序依次为T2至T8组样本完成桩道预备及纤维桩粘接。微拉伸试件制作,于万能试验机进行微拉伸最大载荷测试,记录数据,扫描电镜下观察成型钻的表面形态变化,观察桩道壁表面情况,分析界面破坏方式,对数据进行统计学分析。结果:回归分析显示,成型钻应用频次与桩道各部位纤维桩粘接强度存在明显相关性,冠、中、根1/3部位回归方程分别为y=15.650-0.717x,y=15.463-0.789x,y=15.374-0.886x。8组样本间的粘接强度差异具有统计学意义。T1至T5组间差异无统计学意义。T6、T7、T8与T1至T5任一组之间的差异均有统计学意义。成型钻电镜下观察发现T1至T5组,随着应用次数增加,钻针边缘锐利度降低,但仍清晰完整,T6组至T8组主要变化为局部凹凸不平,边缘出现不连续的小缺损。微拉伸试验根管表面电镜观察发现T1至T5组可见纤维断裂残留部分,可见树脂基质断裂后块状残留,T6至T8组少见上述成分,局部可见树脂突,牙本质小管逐渐清晰可见。结论:纤维桩成型钻应用(高温高压消毒)频次增加会影响纤维桩的粘接强度,建议临床及时更换钻针,保持有效的切削效率,提高纤维桩修复的长期成功率。Objective:To investigate the effects of using frequency of shaping drills in posts on the bond strength about fiber posts.Methods:A total of 48 second premolars which were simple root canal were amputated at 2 mm to the cement enamel junction,and then root canal therapy were performed.All the samples were randomly divided into eight groups,with six samples in every group,which were recorded as T1 to T8.Post space preparation of group T1 were performed using 6 shaping drills whose diameter was 1.5 mm.Ultrasonic cleaning was operated on the shaping drills after the fiber posts were bonded,and then the shaping drills were conducted sterilization cycles in autoclave.The samples of group T2 to T8 were performed following the group T1 procedure.Micro launch test was used to test the bonding strength after the samples were cut into 2.0 mm-thick sections.Morphology of shaping drills and the surface of root canal were observed by SEM.Failure pattern was examined on all the debonded samples.Data was analyzed by statistical software.Results:There is a significant correlation between the bond strength of fiber posts and using frequency of shaping drills by regression analysis.Regression equation of coronal,middle and apical third of post space was y=15.650-0.717x,y=15.463-0.789x and y=15.374-0.886x respectively.Significant differences existed of bonding strength among 8 groups by ANOVA.There was no significant difference in any two groups from T1 to T5 by SNK test,on the other hand,significant differences existed among group T6,T7,T8 and any group in T1 to T5.SEM observation showed the edge of shaping drills was complete in T1 to T5,although the sharpness reduced.Irregularity on the surface and small defect on the edge was observed in T6 to T8.Fibrous and resin matrix residual was observed on the debonded samples in T1 to T5,while rarely in T6 to T8.Conclusion:The bond strength of fiber posts was decreased after multiple use of shaping drills,which should be changed timely to ensure long time success rate of fiber-post resto
