五脏痹理论在类风湿性关节炎多系统受累中的应用探讨

摘　　要：基于理论溯源及临证观察,认为源自《素问·痹论篇》的五脏痹理论与RA多系统受累密切相关。结合五脏痹理论的内涵,认为RA多系统受累初期以“正虚感邪,邪滞五体”,后期以“久病邪深,五脏受损”为病机特点,同时辨析RA多系统受累表现与五脏痹证候的相关性,探讨了以针刺原穴、加用入脏药、因势利导、同调五体痹来调补五脏以及巧用活血通络药、少用风湿痰滞之品、加用导药祛五脏杂糅之邪来治疗RA多系统受累的方法,提示五脏痹理论对RA多系统受累的辨治具有重要指导意义。Based on theoretical traceability and clinical observation,it is believed that the Five-Zang-Bi Theory derived from Su Wen·Bi Lun is closely related to the multi-system involvement of RA.Combined with the connotation of the Five-Zang-Bi Theory,it is believed that the multi-system involvement of RA is characterized by the pathogenesis of“positive and deficient sense of evil and evil stagnation in the five bodies”in the early stage,and“long-term disease and deep evil,and damage to the five internal organs”in the later stage.Through the differentiation and analysis of the correlation between the manifestations of multi-system involvement in RA and the syndrome of five viscera impediments,the exploration has been made in the use of acupuncture at the original point,the addition of viscera-infused herbs,the use of guides according to the situation,the simultaneous adjustment of five viscera impediments to adjust and replenish the five viscera organs,the skillful use of blood-activating and collaterals-dredging medicines,the less use of rheumatism-phlegm-stagnation products and the more use of guide medicine to dispel the mixed evil of the five viscera organs to treat RA with multi-system involvement.The study suggests that the Five-Zang-Bi Theory has important guiding significance for the differentiation and treatment of RA with multi-system involvement.
