摘 要:目的 分析食管鳞癌患者根治术后肺部感染病原学特点及影响因素分析。方法 回顾性分析我院2020年2月至2021年11月收治的128例行食管鳞癌根治术患者的临床资料。统计食管癌患者术后肺部感染率,分析病原学特点及影响食管鳞癌患者术后肺部感染的危险因素。结果 128例食管鳞癌患者,术后发生肺部感染26例(20.31%,感染组),未发生肺部感染102例(79.69%,未感染组),共培养出48株病原株,以革兰阴性菌为主,其次为革兰阳性菌,真菌较少见。两组年龄、吸烟、糖尿病、慢性阻塞性肺疾病、手术时间构成比比较,差异有统计学意义(P<0.05)。年龄、糖尿病、吸烟、慢性阻塞性肺疾病及手术时间为影响术后肺部感染发生的危险因素(P<0.05)。结论 食管鳞癌根治术后肺部感染患者病原菌以革兰阴性菌为主,影响术后肺部感染因素较多,临床应针对病原学特点、危险因素制定相应防治措施。Objective To analyze the etiological characteristics of pulmonary infection in patients with esophageal squamous cell carcinoma(ESCC) after radical resection and influencing factors.Methods Clinical data of 128 patients who underwent radical resection of ESCC in our hospital from February 2020 to November 2021 were retrospectively analyzed. Postoperative pulmonary infection rate in the patients was counted. The features of pathogenic bacteria and risk factors influencing postoperative pulmonary infection were analyzed.Results There were 26 patients(20.31%, infected group) with postoperative pulmonary infection, and 102 cases(79.69%, non-infected group) without pulmonary infection. Forty-eight pathogenic strains were isolated. The pathogenic stains mainly were Gram-negative bacteria followed by gram-positive bacteria. Fungi were less common. There were differences in age, smoking, diabetes, COPD and operation time between the two groups(P<0.05). Age, diabetes, smoking, COPD and operation time were risk factors for postoperative pulmonary infection(P<0.05).Conclusion Pulmonary infection after radical resection of ESCC are mainly induced by gram-negative bacteria. There are many factors affecting postoperative pulmonary infection. Clinical corresponding preventive measures should be formulated according to the etiological characteristics and risk factors.
