摘 要:目的:比较种植支抗和横腭杆支抗对尖牙后移时牙齿和骨骼的影响。方法:回顾性分析2019年2月—2020年12月于汕头市龙湖人民医院就诊的30例Ⅰ类错颌或双颌前突需要拔除第一前磨牙的患者的临床资料。30例患者分为两组,分别使用种植支抗和横腭杆配合Damon Q直丝矫治器进行治疗。对治疗前后的头颅侧位X线片进行测量,比较两组患者牙颌面形态变化的差异。结果:治疗后种植支抗组第一磨牙位置、倾斜度、垂直移动量均无改变,上颌尖牙后移(12.7±1.4)mm、U3-FH角后倾(10.5±1.8)°、U3-PP角后倾(10.5±1.7)°,治疗后横腭杆支抗组上颌第一磨牙近中移动(2.3±0.8)mm、U6-FH角前倾(3.2±1.8)°、U6-PP角前倾(2.3±0.9)°、U6-FH距伸长(1.5±1.0)mm、U6-PP距伸长(1.1±0.6)mm,上颌尖牙后移(10.3±1.8)mm、U3-FH角后倾(12.5±1.8)°、U3-PP角后倾(13.2±1.7)°,两组差异均有统计学意义(P值均<0.05)。结论:在第一前磨牙拔除的治疗中,横腭杆支抗不能提供一个有效的支抗,种植支抗在上颌尖牙后移时提供的支抗更有效。Objective:To compare the effects of temporary anchorage devices(TADs)and transpalatal arch(TPA on teeth and maxillary bones for canine retraction.Methods:The clinical data of 30 patients with Class I malocclusion or bimaxillary protrusion who were treated in Shantou Longhu People’s Hospital from February 2019to December 2020 and required first premolar extraction were retrospectively analyzed.Thirty patients were divided into two groups and treated with TADs and TPA combined with Damon Q straight wire appliance respectively.The lateral cranial X-ray films before and after treatment were measured,and the differences in dental and maxillofacial morphology were compared between the two groups.Results:After treatment,the position,inclination and vertical movement of the first molars in the TADs group did not change.The maxillary canines were moved backward(12.7±1.4)mm,the U3-FH angle was retroverted(10.5±1.8)°,and the U3-PP angle was retroverted(10.5±1.7)°.After treatment,the maxillary first molars moved mesially(2.3±0.8)mm,the U6-FH angle was anteriorly inclined(3.2±1.8)°,the U6-PP angle was anteriorly inclined(2.3±0.9)°,and the elongation of the U6-FH distance was(1.5±1.0)mm,and the U6-PP distance was(1.1±0.6)mm,maxillary canine posterior shift(10.3±1.8)mm,U3-FH angle retroversion(12.5±1.8)°,U3-PP angle backward(13.2±1.7)°in the TPA group,and the difference between the two groups was statistically significant(all P<0.05).Conclusion:In the treatment of the first premolar extraction,the TPA cannot provide an effective anchorage,and the TADs provides more effective anchorage when the maxillary canine is displaced backward.
