摘 要:目的探讨窄带成像联合近焦放大内镜在无蒂锯齿状病变(SSLs)中的诊断价值。方法前瞻性选取2017年1月至2020年12月于许昌市中心医院接受诊疗的100例疑似SSLs患者临床资料,所有入选者均接受窄带成像联合近焦放大内镜检查,以病理检查结果为金标准,分析窄带成像联合近焦放大内镜在SSLs中的诊断价值。结果经病理诊断结果显示,100例疑似SSLs患者中无蒂锯齿状病变83例(83.00%),非无蒂锯齿状病变17例(17.00%);窄带成像联合近焦放大内镜诊断SSLs的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为95.18%(79/83)、82.35%(14/17)、93.00%(93/100)、96.34%(79/82)、77.78%(14/18);窄带成像联合近焦放大内镜诊断无蒂锯齿状病变与病理诊断结果具有极好一致性(Kappa=0.776,P<0.05)。结论窄带成像联合近焦放大内镜在SSLs及性质判定中具有较高应用价值。【Objective】To evaluate the diagnostic value of narrow-band imaging combined with near-focus magnifying endoscopy in sessile serrated lesions(SSLs).【Methods】The clinical data of 100 patients with suspected SSLs who received diagnosis and treatment in our hospital from January 2017 to December 2020 were prospectively selected,all selected patients underwent narrow-band imaging combined with near-focus magnifying endoscopy,the results of pathological examination were regarded as the golden standard,the value of narrow-band imaging combined with near-focus magnifying endoscopy in the diagnosis of SSLs was analyzed.【Results】The results of pathological diagnosis showed that,of the 100 patients with suspected SSLs,83(83.00%)had sessile serrated lesions and 17(17.00%)had non sessile serrated lesions;the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of narrow-band imaging combined with near-focus magnifying endoscopy in the diagnosis of sessile serrated lesion were 95.18%(79/83),82.35%(14/17),93.00%(93/100),96.34%(79/82)and 77.78%(14/18),respectively;narrow band imaging combined with near-focus magnifying endoscopy in the diagnosis of sessile serrated lesions showed excellent consistency with pathological diagnosis results(Kappa=0.776,P<0.05).【Conclusion】Narrow band imaging combined with near-focus magnifying endoscopy has a high application value in the diagnosis and characterization of SSLs.
