SWE联合SGUS评估SS腮腺病变的临床价值

摘　　要：目的探讨剪切波弹性成像技术(SWE)联合唾液腺超声评分法(SGUS)评估干燥综合征(SS)患者腮腺病变的临床价值。方法收集2019年5月-2022年2月在兰州大学第一医院就诊的SS患者112例为实验组,并选取30例健康体检者作为对照组,使用SWE测量受试者右侧腮腺的平均弹性模量值(E_(mean)),使用SGUS评估腮腺内部回声并得出超声评分。通过受试者操作特征(ROC)曲线分别得出腮腺E_(mean)、超声评分及两者联合对SS患者腮腺病变的诊断价值。结果腮腺E_(mean)和腮腺超声评分实验组与对照组间差异有统计学意义(P<0.05)。ROC曲线显示,腮腺E_(mean)、超声评分及两者联合诊断SS腮腺病变的曲线下面积分别为0.873、0.911及0.955。腮腺E_(mean)诊断SS的敏感性为79%,特异性为73%,准确率为78%;超声评分诊断SS的敏感性为81%,特异性为100%,准确率为85%;两者联合诊断SS的敏感性为83%,特异性为100%,准确率为87%。结论SWE及SGUS用于SS患者腮腺病变的诊断均有较高诊断价值,且两者联合诊断的准确率更高。Objective To evaluate the clinical value of shear wave elastography(SWE)combined with salivary gland ultrasound scoring(SGUS)in the evaluation of parotid gland lesions in patients with the Sjogren syndrome(SS).Methods 112 patients with SS treated in The First Hospital of Lanzhou University from May 2019 to February 2022 were collected as the experimental group,and 30 healthy people were selected as the control group.The average modulus of elasticity(E_(mean))of the right parotid gland of the two groups was measured by SWE.The internal echo of the parotid gland was evaluated by SGUS and the ultrasonic score obtained.The diagnostic value of the parotid gland E_(mean),ultrasound score and their combination in patients with SS was obtained by ROC curve.Results There was significant difference in the parotid E_(mean)and parotid ultrasound score between the experimental and the control group.According to the ROC curve,the area under the curve of the parotid E_(mean),ultrasound score and their combined diagnosis of SS parotid lesions were 0.873,0.911 and 0.955,respectively.The sensitivity,specificity and accuracy of the parotid E_(mean)in the diagnosis of SS were 79%,73%and 78%.The sensitivity,specificity and accuracy of the ultrasound score in the diagnosis of SS were 81%,100%and 85%.The sensitivity,specificity and accuracy of the combined diagnosis of SS were 83%,100%and 87%.Conclusion SWE and SGUS had high diagnostic value in the diagnosis of parotid gland lesions in patients with SS,and the accuracy of combined diagnosis was higher.
