摘 要:目的探索年轻乳腺癌患者的临床病理特征与预后生存关系。方法回顾性分析2016年1月-2020年12月在兰州大学第一医院诊治的年龄≤40岁乳腺癌患者的临床病理及人类表皮生长因子受体2(Her2)蛋白不同表达与预后关系。采用Kaplan-Meier生存曲线分析3年总生存时间和无病生存期,Cox回归分析影响患者生存的危险因素。结果134例患者中57例(42.54%)有腋窝淋巴结转移;11例(8.21%)分子分型为Luminal A型、73例(54.48%)为Luminal B型;28例(20.90%)为Her2过表达、22例(16.42%)为三阴性;49例(36.57%)为Her2零表达、57例(42.54%)为Her2低表达。不同分子分型的无病生存期差异有统计学意义(P<0.05)。Her2蛋白不同表达患者的激素受体状态差异有统计学意义(P<0.05)。结论肿瘤直径是总生存时间的独立预后因子;肿瘤直径、腋窝淋巴结转移及分子分型是无病生存期的独立预后因子。在Her2蛋白不同表达的年轻乳腺癌患者中,总生存时间及无病生存期差异无统计学意义(P>0.05)。Objective To explore the relationship between clinicopathological features,prognosis and the survival of young breast cancer patients.Methods A retrospective analysis was performed on the clinicopathology and the relationship between different expressions of human epidermal growth factor receptor 2(Her2)protein and prognosis in breast cancer patients aged≤40 years old who were diagnosed and treated in The First Hospital of Lanzhou University from January 2016 to December 2020.The Kaplan-Meier survival curve was used to analyze 3-year overall survival and disease-free survival,Cox regression analysis was used to analyze the risk factors affecting patient survival.Results 57(42.54%)cases had axillary lymph node metastasis;11(8.21%)had Luminal A,73(54.48%)had Luminal B,28(20.90%)had Her2 overexpression,22(16.42%)were triple negative;49(36.57%)had zero expression and 57(42.54%)had a low expression.The differences in the disease-free survival(DFS)with different molecular types were statistically significant(P<0.05).The hormone receptor status of patients with different Her2 protein expression was compared,the difference was statistically significant(P<0.05).Conclusion Tumor diameter is an independent prognostic factor for the overall survival rate.Tumor diameter,axillary lymph node metastasis and molecular type are independent prognostic factors for disease-free survival.Among younger patients with different expressions of Her2 protein,there should be no difference between the overall survival and disease-free survival.
