摘 要:[目的]研究缺血性脑卒中与自噬、缺血缺氧损伤的关系及自噬流激活对Lp-PLA2影响。[方法]选取98例缺血性脑卒中患者列入观察组,98例健康人群为对照组,观察组内分大动脉粥样硬化型组(LAA)、小动脉闭塞型组(SAA)、心源性栓塞型组(CE)组。采用酶联免疫法测定两组LC3-Ⅱ、Lp-PLA2蛋白水平;采用qRT-PCR测定Beclin-1基因表达量。采用小鼠RAW264.7巨噬细胞,分为OGD、OGD+MA、OGD+RP、对照组,采用蛋白免疫印迹分析测定LC3-Ⅱ以及Lp-PLA2水平。[结果]观察组患者Beclin-1(2.87±2.07)、LC3-Ⅱ(67.47%±12.58%)及Lp-PLA2(295.37±52.71 mg/L)水平高于对照组;不同亚组间三项指标比较,CE组高于SAA组,而LAA组最低,分析显示患者Beclin-1、LC3-Ⅱ与Lp-PLA2水平呈现显著的相关性。细胞试验中OGD组Lp-PLA2水平(158.87±2.58 mg/L)高于对照组(116.39±2.47 mg/L),而显著低于OGD+MA组(299.59±1.75 mg/L)及OGD+RP组(315.20±1.73 mg/L),Beclin-1、LC3-Ⅱ与其趋势相同。以上差异具统计学意义(P<0.05)。[结论]伴缺血性脑卒中疾病进展,患者Beclin-1、LC3-Ⅱ及Lp-PLA2升高,表明自噬及缺血缺氧损伤加重,且自噬流激活可通过促进Lp-PLA2参与疾病进展。[Objective]To study the relationship between autophagy and ischemic hypoxia injury in ischemic stroke and the effect of autophagy flow activation on LP-PLA2.[Method]A total of 98 patients with ischemic stroke were included in the observation group and 98 healthy people were included in the control group.The observation group was divided into major atherosclerosis group(LAA),small artery occlusion group(SAA)and cardiogenic embolism group(CE).Lc3-Ⅱand LP-PLA2 protein levels in both groups were determined by elisa.Beclin-1 gene expression was determined by qRT-PCR.Mice RAW264.7 macrophages were divided into OGD,OGD+MA,OGD+RP and control group.The levels of LC3-Ⅱand LP-PLA2 were determined by Western Blot analysis.[Result]The levels of Beclin-1(2.87±2.07),LC3-Ⅱ(67.47%±12.58%)and LP-PLA2(295.37±52.71 mg/L)in observation group were higher than those in control group.Comparison of three indexes among different subgroups showed that CE group was higher than SAA group,while LAA group was the lowest.Analysis showed that Beclin-1,LC3-Ⅱand LP-PLA2 levels were significantly correlated.In cell test,the level of LP-PLA2 in OGD group(158.87±2.58)was higher than that in control group(116.39±2.47 mg/L),but significantly lower than that in OGD+MA group(299.59±1.75 mg/L)and OGD+RP group(315.20±1.73 mg/L).Beclin-1 and LC3-Ⅱhave the same trend.The differences were statistically significant(P<0.05).[Conclusion]Beclin-1,LC3-Ⅱand LP-PLA2 were elevated in patients with ischemic stroke disease progression,indicating that autophagy and ischemic hypoxia injury were aggravated,and autophagy flow activation could participate in disease progression by promoting LP-PLA2.
