摘　　要：目的探讨颌间牵引钉颌间牵引联合钛板坚固内固定治疗颌骨骨折的临床疗效。方法回顾性分析2018年4月至2020年5月本院收治的66例颌骨骨折患者的临床资料,根据治疗方法不同分为研究组(n=34)与参照组(n=32)。参照组采用钛板坚固内固定治疗,研究组采用颌间牵引钉颌间牵引联合钛板坚固内固定治疗,比较两组手术前后软垢指数、牙龈指数、咀嚼功能评分、颌面部功能评分。结果术前,两组软垢指数、牙龈指数比较差异无统计学意义;术后3个月,两组软垢指数、牙龈指数均高于术前,但研究组低于参照组,差异有统计学意义(P<0.05)。术前,两组颌面部功能评分、咀嚼功能评分比较差异无统计学意义;术后3个月,两组颌面部功能评分、咀嚼功能评分均低于术前,且研究组低于参照组,差异有统计学意义(P<0.05)。结论颌间牵引钉颌间牵引联合钛板坚固内固定治疗颌骨骨折效果显著,可减轻对牙周组织的损伤,显著改善患者咀嚼功能与颌面部功能。Objective To investigate the clinical effect of intermaxillary traction screw intermaxillary traction combined with titanium plate firm internal fixation in the treatment of jaw fractures.Methods The clinical data of 66 patients with jaw fractures admitted to our hospital from April 2018 to May 2020 were retrospectively analyzed,and they were divided into the study group(n=34)and the reference group(n=32)according to different treatment methods.The reference group was treated with titanium plate firm internal fixation,and the study group was treated with intermaxillary traction screw intermaxillary traction combined with titanium plate firm internal fixation,the soft scale index,gingival index,masticatory function score,and maxillofacial function score were compared between the two groups before and after surgery.Results Before surgery,there was no significant difference in the soft scale index and gingival index between the two groups;3 months after surgery,the soft scale index and gingival index in the two groups were higher than those before surgery,but the study group were lower than the reference group,and the difference was statistically significant(P<0.05).Before surgery,there was no significant difference in the scores of maxillofacial function and masticatory function between the two groups;3 months after surgery,the scores of maxillofacial function and masticatory function of the two groups were lower than those before surgery,and the study group was lower than the reference group,and the difference was statistically significant(P<0.05).Conclusion Intermaxillary traction screw intermaxillary traction combined with titanium plate firm internal fixation has significant effect in the treatment of jaw fractures,which can reduce the damage to the periodontal tissue and significantly improve the masticatory function and maxillofacial function of patients.
