LAP、CA125、CA19-9、CEA在子宫内膜癌诊断及分期中的应用价值

摘　　要：目的:探讨血清亮氨酸氨基肽酶(LAP)、CA125、CA19-9及CEA水平在子宫内膜癌(EC)诊断及分期中的应用价值。方法:选取2015年9月至2016年11月在本院就诊的EC患者105例为病例组,同期健康体检者68例为对照组,分别检测其LAP活性、CA125、CA19-9及CEA水平。结果:病例组LAP、CA125、CA19-9高于对照组(P<0.05)。Ⅳ期患者LAP、CA125高于Ⅰ+Ⅱ期患者(P<0.05)。单项检测时,LAP诊断EC的特异性最高为87%,CA125敏感性最高为81%,LAP+CA125+CA19-9联合诊断有较高的灵敏度和特异性,分别为62%、64%。结论:LAP、CA125、CA19-9在诊断EC方面有一定的价值,其中LAP对EC的分期也有一定的应用价值,且诊断特异性较高,LAP+CA125+CA19-9联合检测有较好的灵敏度和特异性,合理选择血清标志物,可协助医生对疾病的轻重程度、肿瘤分期进行判断,从而制定合适的治疗方案。Objective:To investigate the application value of serum leucine aminopeptidase(LAP),CA125,CA19-9 and CEA levels in the diagnosis and staging of endometrial carcinoma.Methods:A total of 105 patients with endometrial cancer who visited our hospital from September 2015 to November 2016 were selected as the case group,A contemporary cohort of 68 healthy subjects was included as the control group.The levels of LAP,CA125,CA19-9 were detected,respectively.Results:The levels of LAP,CA125 were higher in the case group than those in the control group(P<0.05),and were higher in the group of stage IV patients than the group of stage I+II patients(P<0.05).In the diagnosis of endometrial cancer,LAP had the highest specificity of 87%,CA125 had the highest sensitivity of 81%,The combined diagnosis of LAP+CA125+CA19-9 had a high sensitivity and specificity,62%and 64%,respectively.Conclusion:LAP,CA125,CA19-9 has a certain value in the diagnosis of endometrial cancer,LAP also has a certain value for the staging of endometrial cancer,and the diagnostic specificity is high,LAP+CA125+CA19-9 combined detection has good sensitivity and specificity,reasonable selection of serum markers,can help doctors to judge the severity of the disease,tumor staging,so as to develop an appropriate treatment plan.
