超声造影在乳腺导管内病变中的应用价值

摘　　要：目的探讨超声造影在乳腺导管内病变中的应用价值。方法回顾性分析经手术病理证实病变局限于乳腺导管内的50例患者共53个病灶彩色多普勒超声声像图及超声造影特征。结果50例患者的53个病灶中,非肿瘤病灶8个,肿瘤病灶45个(其中良性组28个;恶性组17个)。超声造影后导管内非肿瘤病变多表现为无增强或低增强;恶性组多表现为增强后纵径增大及增强后病灶边界不清晰,相较于良性组差异有统计学意义(P=0.003、P<0.001)。结论局限于导管内的乳腺病灶在常规超声中表现复杂多样,超声造影在诊断乳腺导管内是否有实质性肿物有重要意义,对导管内肿物良恶性鉴别诊断也有一定价值。Objective To investigate the value of contrast-enhanced ultrasound(CEUS)in breast intraductal lesions.Methods Retrospective analysis was performed on 53 lesions of 50 patients who were admitted to the General Hospital of Northern Theater Command from May 2017 to May 2021 and confirmed by surgery and pathology that the masses were limited within the breast ducts.Results Among the 53 lesions,8 lesions were non-tumors and 45 lesions were tumors(benign group:28;Malignant group:17).The non-tumors in the duct after CEUS were mainly manifested as no enhancement or low enhancement.Intraductal malignant lesions were mainly manifested by increased anteroposterior diameter and unclear lesion boundaries after enhancement,which showed statistically significant differences compared with intraductal benign lesions(P=0.003,0.003).Conclusion The manifestations of intraductal breast lesions in conventional ultrasound are complex and varied.CEUS plays an important role in the diagnosis of the masses in the breast duct whetter there are tumors or non-tumours,and is also valuable in the differential diagnosis of benign and malignant intraductal masses.
