摘　　要：目的探讨老年髋部骨折术后并发消化道应激性溃疡的危险因素。方法选取2019年1月—2021年3月重庆医药高等专科学校附属医院接收的255例老年髋骨骨折患者为研究对象,根据术后有无发生消化道应激性溃疡分为发生组和未发生组,统计并比较患者的一般资料,采用Logistic回归分析明确影响老年髋骨骨折术后发生消化道应激性溃疡的因素。结果单因素分析结果显示患者年龄≥79岁、长期使用抗非甾体抗炎药物、既往有消化性溃疡病史、交通事故、麻醉时间≥2 h是患者术后并发消化道应激性溃疡的危险因素(均P<0.05);多因素分析结果显示,长期使用非甾体抗炎药、既往有消化性溃疡病史均是老年髋部骨折术后消化道应激性溃疡发生的独立危险因素(均P<0.05)。结论老年髋部骨折术后并发消化道应激性溃疡的危险因素较多,临床对于具有上述危险因素的高危人群应给予重视并在发生应激性溃疡之前采取相应干预措施。Objective To investigate the risk factors of digestive tract stress ulcer after hip fracture surgery in the elderly.Methods 255 elderly patients with hip fractures received from the Affiliated Hospital of Chongqing Medical College from January 2019 to March 2021 were selected as the research objects,who were treated surgically underwent gastroscopy and were divided into two groups according to whether peptic stress ulcer existed:digestive tract stress ulcer group(n=62)and control group(n=193).Relevant demographical and clinical data were collected.Logistic regression univariate and multivariate analyses were used to determine the factors influencing the occurrence of stress ulcer after hip fracture surgery in the elderly.Results Univariate analysis showed that age≥79,long-term use of nonsteroidal anti-inflammatory drug,history of peptic ulcer,traffic accident,duration of anesthesia≥2 h were the risk factors of postoperative stress ulcer(all P<0.05).The results of multivariate analysis showed that,long-term use of nonsteroidal anti-inflammatory drug,history of peptic ulcer were independent risk factors for stress ulcer after hip fracture in the elderly(P<0.05).Conclusion There are many risk factors of digestive tract stress ulcer after hip fracture in the elderly:age≥79,long-term use of non-steroidal anti-inflammatory drug,history of peptic ulcer,traffic accident,duration of anesthesia≥2 h.
