健脾祛毒汤同步化疗对晚期胃癌患者免疫调节作用的研究

摘　　要：目的探讨健脾祛毒汤同步化疗对晚期胃癌患者的生活质量及免疫机制的调节作用。方法选取收治的95例的胃癌患者,随机进入分为两组,对照组46例,观察组49例。对照组患者给予单纯采用顺铂+曲妥珠单抗+氟尿嘧啶化疗方案治疗,观察组患者在对照组治疗的基础上采用健脾祛毒汤治疗,两组均以28 d为1个治疗周期,化疗2个周期。比较治疗前后两组患者肿瘤标志物(CA242、CEA和CA199)的改变情况;记录两组患者疾病进展时间,平均生存时间;比较治疗后两组的中医证候积分、T细胞的亚群百分比、生活质量及不良反应。结果治疗后观察组的肿瘤标志物(CA242、CEA和CA199)的表达均显著低于治疗前,并且明显较对照组低(P<0.05);治疗后观察组的CD_(3)^(+)、CD_(4)^(+)及CD_(4)^(+)/CD_(8)^(+)表达则均明显高于对照组,观察组的生活质量评分均也较对照组的变化幅度更为明显(P<0.05);观察组患者治疗后的中医证候积分也较对照组降低的更为显著(P<0.05);观察组患者的疾病进展时间、平均生存时间均较对照组患者长(P<0.05);观察组不良反应总发生频次明显低于对照组(P<0.05)。结论健脾祛毒汤可以明显提高晚期胃癌患者生活质量,并且具体机制与调节机体的CD_(3)^(+)、CD_(4)^(+)及CD_(4)^(+)/CD_(8)^(+)有关。Objective To explore the regulating effect of Jianpi Qudu Decoction(健脾祛毒汤)on the quality of life and immune mechanism of patients with advanced gastric cancer.Methods A total of 95 gastric cancer patients were selected and randomly divided into two groups,46 cases in the control group and 49 cases in the observation group.Patients in the control group were treated with cisplatin+trastuzumab+fluorouracil chemotherapy regimen alone,while those in the observation group were treated with Jianpi Qudu Decoction on the basis of the control group.Both two groups received two treatment cycles of chemotherapy,in which 28 d was used as 1 treatment cycle.The changes of tumor markers(CA242,CEA and CA199)of the two groups before and after treatment were compared.The disease progression time and average survival time of the two groups were recorded.The TCM syndrome scores,percentage of T cell subgroups,quality of life and adverse reactions of the two groups after treatment were compared.Results The expressions of tumor markers(CA242,CEA and CA199)in the observation group after treatment were significantly lower than those before treatment,and were significantly lower than those in the control group(P<0.05).After treatment,the expressions of CD_(3)^(+),CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the observation group were significantly higher than those in the control group,and the score of the quality of life in the observation group also changed more significantly than that in the control group(P<0.05).The TCM syndrome scores of patients in the observation group were also significantly lower than those in the control group after treatment(P<0.05).The disease progression time and average survival time of the patients in the observation group were longer than those in the control group(P<0.05).The total frequency of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Jianpi Qudu Decoction can significantly improve the quality of life of patients with advanced g
