摘　　要：目的探讨血液红细胞参数检验在缺铁性贫血中的应用价值。方法纳入2017年1月至2020年10月我院收治的240例贫血患者(分为缺铁性贫血组与地中海贫血组,各120例)与120例健康志愿者(对照组)。比较三组的红细胞参数;分析红细胞参数检验对不同类型贫血的诊断结果;比较不同严重程度、预后缺铁性贫血患者的红细胞参数;分析红细胞参数与缺铁性贫血严重程度及预后的相关性。结果缺铁性贫血组的血红蛋白(Hb)水平及红细胞计数(RBC)、平均红细胞体积(MCV)、平均红细胞血红蛋白含量(MCH)均低于对照组,红细胞分布宽度(RDW)高于对照组(P<0.05);缺铁性贫血组的Hb水平及RBC、MCH低于地中海贫血组,MCV、RDW高于地中海贫血组(P<0.05)。重度贫血患者的Hb水平及RBC、MCV、MCH低于轻中度贫血患者,RDW高于轻中度贫血患者(P<0.05)。预后良好患者的Hb水平及RBC、MCV、MCH均高于预后不良患者,RDW低于预后不良患者(P<0.05)。Hb、RBC、MCV、MCH与缺铁性贫血严重程度呈负相关,与预后呈正相关(P<0.05);RDW与缺铁性贫血严重程度呈正相关,与预后呈负相关(P<0.05)。结论血液红细胞参数作为缺铁性贫血的诊断指标,还可对患者病情、预后进行准确评估和预测。Objective To investigate the application value of blood red blood cell parameter test in iron deficient anemia.Methods A total of 240 anemia patients(divided into iron deficient anemia group and thalassemia group,with120 cases in each group)and 120 healthy volunteers(control group)admitted in our hospital from January 2017 to October 2020 were included.The red blood cell parameters of the three groups were compared;the diagnostic results of red blood cell parameter test for different types of anemia was analyzed;the red blood cell parameters of iron deficient anemia patients with different severity and prognosis were compared;the correlation between red blood cell parameters and severity and prognosis of iron deficiency anemia were analyzed.Results The hemoglobin(Hb)level,red blood cell count(RBC),mean corpuscular volume(MCV)and mean corpuscular hemoglobin(MCH)in the iron deficient anemia group were lower than those in the control group,and the red blood cell distribution width(RDW)was higher than that in the control group(P<0.05);the Hb level,RBC and MCH in the iron deficient anemia group were lower than those in the thalassemia group,and MCV and RDW were higher than those in the thalassemia group(P<0.05).The Hb level,RBC,MCV and MCH of severe anemia patients were lower than those of mild and moderate anemia patients,and RDW was higher than that of mild and moderate anemia patients(P<0.05).The Hb level,RBC,MCV and MCH of patients with good prognosis were higher than those of patients with poor prognosis,and RDW was lower than that of patients with poor prognosis(P<0.05).Hb,RBC,MCV and MCH were negatively correlated with the severity and positively correlated with the prognosis of iron deficient anemia(P<0.05);RDW was positively correlated with the severity and negatively correlated with the prognosis of iron deficient anemia(P<0.05).Conclusion As a diagnostic index of iron deficient anemia,blood red cell parameters can also accurately evaluate and predict the patient’s condition and prognosis.
